Update on Group Consultation Training
A General Practice Nurse
10 Point Plan Initiative

 
Prior to the Covid-19 pandemic, Devon Training Hub was planning a pilot training programme for 10 practices interested in delivering group consultations. Unfortunately this programme was paused - but now we have a fantastic new opportunity open to ALL practices! 
 
The Experience Led Care Programme Team have been commissioned by NHSEI to run a national training scheme for Virtual Group Clinics. Here in Devon we are hosting three briefing sessions introducing Virtual Group Clinics. 
 
This training is open to GPNs, Practice & Network Managers, PCN clinical directors, GPN leads and others who have interest in finding out more about virtual groups clinics across Devon. The sessions will discuss the benefits to patients and staff and the practicalities of running groups. The sessions will be recorded for people to view later if they cannot join live.
 
Practice teams can then go on to join the national scheme which provides 2.5 hours of free online training. As part of a GPN 10 Point Plan initiative, local participants can receive additional training and expert support to start their own groups.
 
The dates of the available sessions are as follows:
 
Tuesday 7th July 2020 - 1pm-2pm – Book now
Wednesday 8th July 2020 - 2pm-3pm – Book now

 
NB - you are only required to attend ONE session

As many of our regular delegates will be aware, since the dramatic changes imposed on 23rd March we have been using Zoom as the platform for our online forums and webinars.  Up to now we have found this to be the most user-friendly and accessible platform, but to ensure that it remains fit for purpose we have put some security measures in place.

We appreciate that not everyone will have had the opportunity to use Zoom before coming to one of the forums, so we have written a Delegate's Guide to help people through the process.  Please find the guide here.

When you have booked a course with the Devon Training Hub, we will send you an email link to register for the meeting in Zoom about three days before the event.  This ensures that only registered guests are able to join us for the meeting.  If you have not used Zoom before, you might have to create a free account with Zoom first, then go back and register for the meeting.  Please be aware that you will have to register for every course that we host on this platform, but you will only have to create an account with Zoom once.   

We recommend that your Zoom account uses the same email address as your Devon Training Hub account so that your details match those on our meeting guest lists.

How to Get Started: Care Home Medicine, Communication & the MDT

Thursday 9th July 12.30-13.30 

 

Primary care networks (PCNs) across Devon are at very different stages of working with care home teams. Different priorities and challenges arise in delivering the Enhanced Health in Care Homes (EHCH) specifications and there is no ‘one size fits all’ solution.

 

This one-hour webinar will highlight a variety of approaches for multidisciplinary teams (MDTs) to work with care homes. We will hear from both the PCN and GP perspective, sharing a range of experiences in collaborating with care homes, setting up MDTs, and putting theory into practice. They will share their experience, challenges and learning from delivering proactive, joined up care.

 

This webinar is primarily for GPs, Nurses, Health and Social Care Teams and welcomes anyone interested or involved in the design and delivery of the new care home specifications set out in PCN Directed Enhanced Services (DES). This webinar aims to support collaborative multidisciplinary learning for the benefit of residents and staff of care homes.

A New Tradition - Annual Clap for the NHS
Sunday 5th July 2020

People are being encouraged to celebrate the NHS’s 72nd birthday and thank key workers for their support during the coronavirus pandemic with a nationwide clap next month.

NHS England boss Sir Simon Stevens and the Archbishop of Canterbury Justin Welby are among the influential figures supporting the initiative.

The initiative would make 5 July an official day to pay tribute.

Clap for carers founder Annemarie Plas is also backing the campaign.

Dutch-born Londoner Ms Plas was credited with starting the nationwide applause for NHS staff and key workers, which saw millions of people clapping at their windows and on their doorsteps on Thursday evenings in lockdown.

In a letter, compiled by the Together coalition, public figures including Sir Simon and the Most Reverend Justin Welby, the leader of the Church of England, voiced their support.

It said: “We all owe a debt of gratitude to the nurses, doctors, physios, porters, cleaners, and countless others who have delivered for patients and their families along with all those in the care sector.

“But we are also hugely grateful to the shop workers, transport staff, delivery drivers, teachers, refuse collectors, farmers, armed service personnel and other key workers who have kept the country going.”

Sir Simon added he wanted to thank the public, in particular, whose support had meant so much – “from the children who put rainbows and NHS signs in their windows, to all those who saved lives by staying at home to slow the spread of this terrible virus”.

RAISING AWARENESS TO REDUCE INSULIN ERRORS

Following a phenomenally successful, Insulin Safety Week (ISW) 2019, the campaign is back. Insulin Safety Week 2020 will take place across the UK and Ireland, raising awareness in a bid to reduce insulin errors among healthcare professionals.

The campaign aims to raise awareness of insulin safety among healthcare professionals. It will take place from July 6 to 12 and follow the same format as Hypo Awareness Week. Register and you will also receive a printed resource pack, including posters, balloons and t-shirts to bring your campaign to life as well as an online resource pack containing graphics, templates and guidelines.

A total of 343 sites took part in the inaugural, nationally-organised ISW last year, which built on interest generated by successful local campaigns across the country, including in Leicester, Hastings, Eastbourne and Southampton. Interest in the campaign increased in 2019, with 531 sites taking part.

This project has been funded by Sanofi who have had no input into any arrangements or content.

COVID19 UPDATES

Latest guidance on identifying COVID-19 in children

Children can get coronavirus (COVID-19), but they seem to get it less often than adults and it’s usually less serious.  The main symptoms of coronavirus are:
  • A high temperature
  • A new, continuous cough
  • A loss or change to sense of smell or taste
If a child has symptoms they should:
  • Stay at home (self-isolate) do not leave home or have visitors.  All members of the household and support bubble must also self-isolate.
  • Get a test to check if they have coronavirus as soon as possible.  All members of the household and support bubble must also get a test.
For children under 5 years old, NHS 111 online should be contacted in the first instance, or NHS 119 if the child’s parent or guardian are unable to access the internet.
 
All acute hospitals in Devon and the testing facilities in Plymouth (Seaton Barracks) and Exeter (Honiton Road Park & Ride) can offer testing facilities to children under 5 years old.  If there are any issues, please contact your local coordination centre (see attached form). 

 
Continuing postnatal checks

As we restore critical services, It's important practices continue providing all women with a postnatal GP check at 6-8 weeks.  During the pandemic, women have experienced changes to the provision of maternity and health visiting care, reduced access to community services, changes to birth plans and birth partner policies, and for some, changes to bereavement care. COVID-19 has increased anxiety generally in the population and pregnant women and new parents may be under additional stress due to social isolation, decreased social support, bereavements, financial stresses, changes to healthcare services and limitations on their usual coping strategies. The risks for pregnant women with pre-existing mental health problems will be higher still.

Since 1 April the maternal postnatal check has been part of essential services. The continuation of postnatal checks, new baby checks and childhood vaccinations have been identified as high priority in BMA and RCGP guidance, and should continue regardless of the prevalence of COVID-19 for the duration of the pandemic.

Where the maternal check can be delivered safely on a face-to face-basis this should continue. Where a check cannot be delivered safely face-to-face, or where the woman has medical conditions that requires her to shield or socially isolate, the review could be conducted remotely either by video or phone. If there are outstanding issues after this, a face to face appointment could be offered at the same visit to the practice as the infant vaccination appointment. A postnatal check must be offered to mothers who have experienced stillbirth or neonatal death.
New face covering journey assistance cards
Practices have reported being approached by some of their patients for confirmation of medical exemption for face coverings on public transport.  Stagecoach is encouraging passengers who are unable to wear a face covering on the bus due to a health condition or a disability to apply for new ‘face covering journey assistance cards to show to drivers when they board to confirm they are exempt from the legislation.
 
There is also a special card available if people need the driver to lower their face covering because they are hearing impaired and need to lip read.
REMINDER: procurement survey for PPE

Devon CCG is keen to organise a collaborative procurement of recommended PPE for CCG commissioned care at GP practices across Devon. Our aim is to procure enough PPE for three months and practices will need to use these supplies rather than order themselves. To get this right we need to obtain some basic information from you, so that we can get you what you need.
 
We have created a short survey for you to fill in.  Please complete this survey based on your expected need over the next three months. You will also need to consider storage of PPE, as its likely this will need to be held by practices.
 
To ensure that we include your estimates in the procurement, please respond by Friday 3 July.
 
The CCG is proposing this as an interim measure while long terms options are considered.  In the meantime, please continue to source PPE independently until these arrangements have been put in place. GP practices will be kept informed about progress. Clipper and the CCG PPE Cell can continue to supply emergency provisions.

  • Thursday 9 July, 3pm or Friday 10 July, 11am Primary care video library onboarding webinar. HCI are inviting NHS staff to join onboarding webinars which will provide an introduction to the video library and how to enable staff to maximise the use of videos right across the health system.


NHS boosts support for pregnant black and ethnic minority women
The NHS is rolling out additional support for pregnant Black, Asian and Ethnic Minority (BAME) women, as new research shows heightened risks facing women from minority groups. Recent analysis shows Black pregnant women are eight times more likely to be admitted to hospital with Covid-19, while Asian women are four times as likely. 
Urgent action is being taken by the NHS in England to protect expectant mums, including increasing uptake of important Vitamin D and undertaking outreach in neighbourhoods and communities in their area.
 
Women from ethnic minority backgrounds have long been known to face additional maternity risks, with maternal mortality rates significantly higher than for white women. But now research from Oxford University shows that 55% of the pregnant women admitted to hospital with Covid-19 are from a BAME background, even though they only make up a quarter of the births in England and Wales.  NHS England has written to all maternity units in the country calling on them to take four specific actions which will minimise the additional risk of Covid-19 for BAME women and their babies.
 
The steps include:
  • Increasing support of at-risk pregnant women – e.g. making sure clinicians have a lower threshold to review, admit and consider multidisciplinary escalation in women from a BAME background.
  • Reaching out and reassuring pregnant BAME women with tailored communications.
  • Ensuring hospitals discuss vitamins, supplements and nutrition in pregnancy with all women. Women low in vitamin D may be more vulnerable to coronavirus so women with darker skin or those who always cover their skin when outside may be at particular risk of vitamin D insufficiency and should consider taking a daily supplement of vitamin D all year.
Ensuring all providers record on maternity information systems the ethnicity of every woman, as well as other risk factors, such as living in a deprived area (postcode), co-morbidities, BMI and aged 35 years or over, to identify those most at risk of poor outcomes.
Preliminary trial results find antiviral drug lopinavir/ritonavir does not reduce death in hospitalised patients with COVID-19

The ‘RECOVERY’ trial has released preliminary results showing lopinavir/ritonavir, an antiviral drug commonly used to treat HIV, had no significant mortality benefit in hospitalised COVID-19 patients.

What if there's a coronavirus outbreak in Devon?

We've developed new measures to contain and reduce cases of coronavirus in Devon, and safeguard the health of our residents. 

It's called the Local Outbreak Management Plan and it details how we will work together to help reduce the risk of outbreaks in the county, and if they do occur, identify them quickly and provide a rapid response to prevent the virus spreading. 

Designed to support the national NHS Test and Trace Service, it is our local blueprint for action and builds on the effort everyone has made so far to keep infection rates here so low, protect each other and support the most vulnerable in our communities.

We are also helping other councils with their localised planning for coronavirus outbreak control as one of 11 authorities chosen by the government to share good practice across the country. 

We've published the plan on our website, and it will be updated as new national guidance is published or legislation changes. You can also download a copy.

I would very much like to hear from as many people as possible, whatever your job role is within Primary Care.  The project I am currently working on will involve me talking with many school age children between 16 and 19 years old.
 
This generation could be the future workforce for Primary Care and I want to provide them with as many positives and reasons why healthcare is a great career choice as I can.

 Each session starts with 30 minute webinar to explain process and help ensure things go smoothly.
 Conversations with practices will be by pre-booked appointment
 Each 15min appointment allows for 12 mins conversation between yourself and practice representative/s and time for smooth change over
 No limit on how many appointments you can book, session times permitting
 There will be a half time interval
 When making an appointment for conversation you will be invited to supply practices with details about yourself ahead of the event if you want to

After the event you will be able to progress with practices of your choice and do so with continuing advice and support from Somerset LMC and Training Hub as required.

As many families continue with home schooling there are lots of children who do not have access to devices or smartphones to support their learning.

Do you have old devices which you no longer need? 

The Ivybridge Rotary Club has set up a project to upgrade previously owned laptop and tablet computers so that they can be made available on a free-loaned basis to support local families with children who can’t otherwise easily access a completer whilst at home during the present pandemic and the recovery period afterwards.
