Message from the CTSI Director

Having just returned from the annual in-person meeting of the Steering Committee of the CTSA consortium and the Fall CTSA Program Meeting in Washington, I am energized by the opportunities that our CTSI has to participate in and contribute to cutting-edge approaches in the consortium through the innovative work of research teams at our CTSA hub.
For example, the NIH is encouraging researchers to use of the Fast Healthcare Interoperability Resource (FHIR) to capture, integrate, and exchange clinical data for research purposes and enhance capabilities to share research data. FHIR, which is widely used in the clinical world, has the capability to dramatically improve the power and interoperability of EHR data for research purposes. Cerner, the EHR platform that is being adopted by Kaleida, ECMC and UBMD has FHIR capability, providing an enormous opportunity to advance clinical research by our research teams in the coming months.
The FDA is placing increasing attention on using “real world data” and “real world evidence” to assess effectiveness of new interventions when they are moved into broader populations, beyond the well-defined inclusion criteria of clinical trials, which show efficacy. Considerations such as adherence, co-morbidities, tolerance, complexity of dosing regimens and others may reduce the effectiveness of a new drug that is efficacious in clinical trials. Research teams should anticipate this issue early in developing plans to move a new intervention along the translational pipeline.
A particularly enjoyable session at the Program Meeting was the opportunity to present a poster outlining our Translational Imaging Core as part of the UB Center for Biomedical Imaging at a session that highlighted “optional modules” at CTSA hubs. The poster generated excellent discussions, particularly the plan to work with other CTSA hubs in creating a platform to share protocols and data. The CTSA consortium and NCATS (NIH) place a high priority on this type of collaborative activity, which is a strength of our CTSI. It is a privilege to represent our CTSI on this national stage.
