We have worked with the Patients' Association and Bournemouth University to develop the Nutrition Wheel. The wheel is an interactive version of the Patients' Association Nutrition Checklist and has been designed to be a conversation starter.
Please note: the Nutrition Wheel itself is made from durable coated card, and as such, Nutrition Wheels can be used many times. The Nutrition Wheel is a tool to be kept by the volunteer or care worker. It is not a patient resource, and as such is not appropriate to give out to patients or clients.
Expressions of Interest: we're looking for a Primary Care Network
Our Primary Care Innovations programme is looking for a Primary Care Network to take part in a trial and evaluation of a digital innovation to assist practices in ensuring they are compliant with CQC standards.
The innovation – QuiqCare - provides a real-time view across multiple locations, which helps to monitor and address compliance. The system can help produce action plans, which can help with continuous improvement. For example, a group of GP practices can monitor which surgeries are complaint with CQC requirements, and which need to undertake actions. For more about the product, visit their website.

ESCAPE-pain rehabilitation comes to Portsmouth
In Portsmouth it is estimated that twenty-two thousand people aged 45 or over have knee or hip osteoarthritis.
This includes sharing patients’ records between doctors and other care providers, installing WiFi at GP surgeries and NHS buildings, and helping local NHS IT systems ‘talk’ to one another better.
NHSE&I are providing a comprehensive programme offer, the SDM Accelerator programme, to support NHS organisations to embed Shared Decision Making into one or more clinical pathways.
This programme has been developed to align to the expansion of Universal Personalised Care, as set out in the Long Term Plan. Its overarching objective is to build capacity of NHS organisations to embed SDM through completing the SDM Self-Assessment checklist and then develop a robust plan for improvement. The programme will help to ensure you can take necessary steps to embed and implement shared decision making across your service in order that people are supported to: understand the care, treatment and support options available and the risks, benefits and consequences of those options, and make a decision about a preferred course of action, based on evidence-based, good quality information and their personal preferences.
The programme will be led and facilitated by the national SDM team and will support individuals and organisations wishing to implement SDM at scale to a high standard. It starts in September 2019 and comprises of 2 full days plus 3 webinars (2 x 90 mins, 1 x 60 mins). All organisations attending these days are strongly encouraged to ensure attendance from a team of people from across the organisation including a clinical lead, a patient leader, a project/programme/QI manager and an executive sponsor, all of who will play a role in embedding SDM into the chosen clinical pathway. Please note than having only 1-2 of these individuals at the events may impact on the effective implementation of the programme in your organisation.
