The Spyros disaster - Singapore - 76 Souls lost, 96 injured #maritimehistory #OTD

The Spyros disaster was a major industrial disaster that occurred in Singapore in October 12, 1978, where the Greek tanker Spyros exploded at Jurong Shipyard, killing 76 people and injuring 69 others.
It remains the worst accident, in terms of lives lost, in Singapore's post-war history. It is also Singapore's worst industrial accident.
Explosion
At around 2:15 p.m., Spyros exploded. The explosion occurred as about 150 workers, including women, returned to the engine and boiler rooms of the vessel after their lunch break for repair and cleaning work. A number of the ship's 32 crew were also on board.
The blast flung debris from the 35,600-tonne vessel as far as 100 metres (328 ft) away and started a flash fire that prevented dockside workers from rescuing those trapped inside the ship.
Rescue works
Minutes after the accident, an extensive rescue operation swung into action, involving the police, military and medical services. All hospitals were alerted to be on stand-by for casualties.
Rescue workers went into the ship's engine and boiler rooms to search for missing workers. Eight fire engines and ambulances rushed to the scene and after the fire was doused, more rescue workers poured into the ship to help the injured and remove the dead. The injured were ferried to hospital in ambulances and helicopters.
Casualties
The wounded were ferried by ambulance and helicopter to Alexandra Hospital and Singapore General Hospital. Most of those warded suffered serious burns, with their conditions described as critical. Others, including four firemen, were treated for inhalation of toxic gas and shock.
In summary, 57 of the 76 died on the day of the accident. The others succumbed to their injuries in hospital. Of the 76 deaths, 70 were male, six were female, and 69 others were injured.
Causes of disaster
An inquiry found that safety practices were ignored when repairs were carried out on the vessel. Sparks from the cutting torch used during repairs caused a fire, which ignited an explosive vapour mixture within the aft starboard bunker (fuel) tank of the vessel. The fuel tank had been contaminated by crude oil. The explosion ruptured the common bulkhead between the tank and the engine room, releasing the burning oil into the engine room and setting it on fire, killing the workers there instantly.
Soon after the accident, greater safety consciousness in the shipbuilding and repairing industry was enforced.

Posn: 18:33.9N – 072:23.2W, Port Au Prince Anchorage, Haiti.
During routine rounds duty AB onboard an anchored ship was taken hostage by robbers armed with guns and long knives. Alarm raised and crew mustered.
Seeing the alerted crew the robbers escaped with stolen ship’s properties. Incident reported to Pilot Station.

08.10.2019:
2240 UTC: Posn: Pointe Noire Anchorage, Congo
Master onboard an anchored research vessel heard a sound near the forward and noticed one unauthorised person onboard. Master shouted and the person escaped.
Emergency alarm raised and crew mustered. A boat was seen departing the vessel with stolen ship's store.

05.10.2019:
1800 – 2359 UTC: Posn: 01:18.14N – 104:07.03E, Johor Anchorage, Malaysia.
Unnoticed robber, boarded an anchored tanker, stole ships properties and escaped. Duty crew on routine rounds noticed the theft and notified the OOW.
120-19
05.10.2019:
0200 UTC: Posn: 06:17.2N – 003:13.8E, Secure Anchorage Area, Lagos, Nigeria.
Duty security patrol onboard an anchored product tanker noticed three robbers on the forecastle and immediately informed the OOW, who raised the alarm. Upon hearing the alarm, the robbers escaped without stealing anything.
Incident reported to Nigerian Navy and a patrol boat arrived for investigation.
119-19
30.09.2019:
0655 UTC: Posn: Callao Port, Peru.
Robbers boarded an anchored bulk carrier. They took hostage the duty AB and tied his hands. When the duty officer did not receive a response from the duty AB he order other crew to investigate. Upon arriving at the forecastle, the crew saw the duty AB, who raised the alarm. hearing the alerted crew the robbers escaped.
The duty AB was unharmed.
Two mooring ropes were reported missing. Incident reported to the port authority and local agent.

UKMTO Weekly Piracy Report #piracy #marsec @UKMTO

1. This week UKMTO issued no advisories or warnings.
2. Vessel operators and ships masters are reminded to register their ships transits of the Strait of Hormuz (SoH) at least 72hrs ahead of their passage. Registering SoH transits with UKMTO and remaining cognisant of the latest Industry, Government and Military guidance remains vital to mitigate against potential risks in the area.
3. On 11 Oct, the UK Department for Transport (DfT) released Security Notification 2019/29 to Red Ensign Group (REG) shipping for transits of the Red Sea North. All REG ship’s CSO should ensure that they are fully aware of the latest Flag State guidance and that it is communicated in a timely manner to ship’s masters.
4. Commander Maritime Forces (CMF) warships HMS MONTROSE and FS JEAN BURT seized 170 kilograms of illegal drugs during operations in the Arabian Sea. For further details see link at para. 5(a).

Maritime Crime Activity
05/10/2019
220 / 8 Nm Lagos
On the 05th October, while anchored in Lagos secured anchorage, three persons boarded the tanker and were immediately spotted by the duty team. As soon as alarm was raised, the persons escaped jumping over board. Tanker informed local authorities and a nigerian patrol boat investigated. Nothing found in the area, crew and tanker are safe.
Maritime Crime Activity
06/10/2019
Lagos, Nigeria
On the 06 OCT 2019 at 2230Z as the ship was berthed and discharging it’s cargo in APAPA BOP TERMINAL a tanker was attack by two wooden boats manned with approx. 7/8 persons. The intruders were approaching in order to steal the cargo.
The crew reacted immediately and suspended the discharging ops. During their escape from the jetty one of the robbers shot at the crew with a firearm. Few hours later, on 07 OCT 2019 at 0212Z a second attempt took place.
Once again two wooden boats with approx. 4/5 persons on board approached the tanker. The crew and jetty security guards took action, at 0230Z the thieves to retreated. No crew member was injured and the vessel was reported safe.
