Survey on Usage of Medical and Mental Health Support Services

We receive calls every week from parents and teachers asking for referrals to qualified professionals for educational evaluations, mental health support or management of medications.  Often parents don’t want to explore this topic publicly and find it very stressful and confusing to identify the qualified support resources they need. 

We want to do more to help parents seeking to support a child, or themselves with learning or attentional issues by pooling our collective experiences and helping to identify qualified medical and mental health practitioners.  If you have used the services of a medical or mental health professional within the past three years, please take three minutes to respond to our questionnaire. 

All individual responses will be kept confidential. Survey results will not be shared publicly but anyone seeking a referral may contact FOCUS and benefit from our collective efforts. 

We encourage you to share this survey link to others you may know who have sought the help of medical or mental health professionals to support a child, adolescent or adult with main stream learning differences and related mental health issues.
