Long Island Population Health Summit
In partnership with the Long Island Association, the Long Island Health Collaborative is hosting a Population Health Summit on Friday, February 28th, 2020.
 

What is it?

The purpose of this event event is to offer participants an inside look at a working value-based purchasing (VBP) arrangement, noting the interdependence of these contracts and the social determinants (SDH) of health.
 

Who should attend?

Anyone who has an interest in ensuring that affordable, quality healthcare is accessible to all Long Islanders.

This includes small to large-sized businesses that purchase insurance, and who may also be interested in developing VBP arrangements with their insurers, to healthcare and medical suppliers and manufacturers, to anyone working in healthcare and social services and public health – community-based organizations, hospitals, health departments, health plans, etc.
 

Why should I attend?

Hear from keynote speaker, Ryan P. Ashe, Director of VBP & Healthcare Innovation at the NYS Department of Health, learn the ins and outs of value-based purchasing from real-life experiences, all at no cost to you or your employer!

Chronic Disease Self-Management Workshops
Help your patients better manage their chronic illnesses by hosting a 6-week workshop series at no cost to your organization! The Long Island Health Collaborative is offering FREE Chronic Disease Self-Management Workshops Throughout Nassau and Suffolk Counties.

The highly-effective and evidence-based Chronic Disease Self-Management Education (CDSME) workshop series, developed at Stanford University and tested for its efficacy, is a valuable FREE service you can offer your patients and/or employees right on site.

CDSME is being offered in the counties of Nassau and Suffolk under the sponsorship of the Nassau-Suffolk Hospital Council (NSHC) and it's Long Island Health Collaborative. NSHC and the Long Island Health Collaborative are absorbing all costs associated with the workshop series.

CDSME is a six-week interactive workshop that provides hands-on interactive and fun education for people and their families or care partners who are living with chronic conditions. The program covers topics such as healthy eating, easy-to-follow physical activity exercises, stress management, medication management, and improving communications with their healthcare team among others in a supportive environment.

Workshops are once a week for 2.5 hours at your location. Participants will receive a Certificate of Achievement, the book Living a Healthy Life with Chronic Conditions and a gift bag.  All at no cost to your organization or to the participant.  If held at the workplace for employees, it is a one-hour workshop for two days.

Cancer Peer Education Program
Free Breast and Prostate Cancer Education
Free Presentation on Breast and Prostate Cancer Education in English or Spanish!

The Cancer Peer Educators at Peconic Bay Medical Center provide free education to men and women, ages 50-74.

Educators can help individuals:
  • Find a primary care physician
  • Find a mammography facility
  • Obtain health insurance
  • For those who are uninsured, access to free cancer screenings
  • Access to community resources such as food pantries, housing, and/or clothing
  • Obtain interpreter services

Project Warmth
Project Warmth is a safety net for financially struggling individuals and families who are unable to pay their heating bill. This program provides a one-time grant for fuel and/or fuel-related electricity. Payments are made directly to oil and utility companies on behalf of eligible residents facing heating emergencies. For the hundreds of families we help each year, assistance with an oil delivery or utility arrears means they have room in their budgets to cover essentials like food or medication.

If you or someone you know needs emergency heating assistance, call United Way’s 2-1-1 Long Island information and referral call center by dialing 2-1-1 (or 1-888-774-7633) 7 days a week, 9:00am-5:00pm, for referrals to an intake site.

Association for Mental Health and Wellness
Free Mental Health First Aid Trainings
#MHStrong, a program of the Association for Mental Health and Wellness (MHAW), is offering free training in Mental Health First Aid. This 8-hour evidence-based course teaches participants how to assist someone who is experiencing mental health distress or developing a substance use problem. The program is funded by a grant from the Substance Abuse and Mental Health Services Administration (SAMHSA) to promote mental wellness.

Scholarship Opportunities
EmPOWERED to Serve Scholars 
Students working to improve health in their communities.

Summary:
EmPOWERED to Serve Scholars are college freshmen, sophomores and juniors making a difference in the health of their communities.  Past scholars include:

  • A health policy and management student who encouraged corner groceries to sell fresh, healthy foods,
  • A biochemistry student who taught diabetes courses at a community health clinic, and 
  • A public policy student who helped launch an urban garden where families living in a food desert could harvest fresh fruit and vegetables.
Ten students will each receive a $10,000 scholarship towards their college tuition.

Capstone Seminar: Population Health Issues

The Program in Public Health is looking to partner MPH candidates with organizations in the process of applying to grant opportunities.

A small team (3-5 students) will work together to complete your grant application before whatever deadline you have! The course starts on Thursday, January 30th and ends in mid-May. There will be two opportunities to speak to the class to explain your organization’s mission and the grant.
