A Successful Post Implementation Review!

The Department of Health and the Office for People with Developmental Disabilities conducted a Post Implementation Review of LIFEPlan on December 19, 2019. 

The Review was to ensure that LIFEPlan was meeting the many requirements for IDD Health Homes in New York State, and entailed a survey of member records as well as an assessment of LIFEPlan's policies and procedures, and quality management plan. 

A great deal of focus was on the medical and behavioral health aspects of care management, including follow-up after transitions of care (e.g. ED visit, hospitalization).

We are proud to report that the review team was very impressed with how far we have come in 18 months as a CCO Health Home, and with our plans to continue developing and improving our systems and processes going forward. After submitting a few additional documents to support our responses, LIFEPlan achieved a score of 100% on its review!!! 

Congratulations go out to our Post Implementation Review Prep Team, to all our care management staff and leadership, and to our Provider Partners for a job well done.

  Easing the Enrollment Process
Meet Monalisa Fermin-Fennell, Care Connection Engagement Specialist

Providing the best member experience is a driving force behind LIFEPlan’s mission. We know that the journey through the eligibility and enrollment process can be confusing and even seem daunting at times. 

To make enrollment easier, LIFEPlan has added a new role to assist prospective members and families with the process. Monalisa Fermin-Fennell is the new Care Connection Engagement Specialist.
