Dear LIHC Participant,
There is a unique healthcare/housing value-based project occurring on Long Island, and we want you to hear all about it – literally – from the members of the project team. Leave inspired and with helpful information and tips to apply to your own value-based arrangement efforts.
At the Long Island Population Health Summit on February 28th, you will have the chance to hear each team member tell their story and relay their experiences with the “housing first” pilot project. This will not be an ordinary panel presentation.
Those involved in this housing project, each panelist, will speak candidly about their experience – simply tell their story. Panelists will share their lived-experience, recalling obstacles they overcame and are perhaps still encountering, how they chose metrics and the parameters for the project, how they worked through the details of the contract, etc. On the panel will be the representative from the commercial insurer involved, the primary care provider, and the representative from the community-based organization that is overseeing the housing piece. There will be no powerpoints – just conversation and questions.
The afternoon summit opens with remarks by Ryan Ashe, director of value-based payment (VBP) and healthcare innovation for New York State. He will discuss the state’s grand vision for VBP and then the panel presentation follows.
The Long Island Population Health Summit is FREE, but registration is required.
Our Long Island universities that offer master programs in public health and/or health administration will be on hand to offer more information about their programs and the special projects assistance graduate students can provide to your organization at no cost.
