Thank you for being part of the Community Data Program. In order to continue to serve our members through access to data, delivery of program resources and tools, and improving our network, we ask that you complete a short survey based on your experience with the program. The survey should only take 5 minutes of your time. We made an effort to only ask questions that help us deliver the program, so we are not wasting your time.
