- Learning Activities Update
- The 12 teams from 10 organizations are continuing to develop and test new workflows, stratify their depression and diabetes populations of focus and report monthly on identified metrics.
- General Updates
- All members of the WPCC Learning Community are fully funded for 2020 and are continuing to implement their change plans.
- There is a Buprenorphine/MAT Waiver training on February 7 at the Confluence Technology Center in Wenatchee. Register here.
- January WPCC Meeting outlined the purpose of the WPCC meeting and attendees chose to address Social Determinants of Health screening process and identifying gaps in care.

Pathways HUB
Visit page
- Local HUB project leads continue to participate in cross-ACH HUB Collaborative meetings, focusing on quality improvement efforts.
- A training for Pathways Community Specialists will be hosted in Moses Lake in February 2020.
- Conversations and plans are continuing for expansion to other counties in region, including recruiting for new service agency partners.

Opioid Project

- The Opioid Workgroup Steering Committee met in January:
- Created an RFP Evaluation Committee for the Opioid Awareness and Education RFP that closed January 24.
- Brainstormed ideas to improve the Targeted Narcan Training Distribution process for distributing funds. Based on feedback, NCACH staff will develop a proposal and bring it to the Workgroup for review in February.
- Since an Elevate Recovery Advocacy Training was provided on January 4 in Wenatchee for no charge, funding allocated that was allocated for this training was adjusted to supporting additional Recovery Series videos.

Transitional Care and Diversion Interventions

- The TCDI Workgroup met on January 16. The following items were covered during the meeting:
- The workgroup moved to approve the recommendation that the NCACH Board support the Community Partnership for Transitions Solution (CPTS) proposal. The CPTS proposal will be brought to the Board in February.
- Columbia Basin Hospital and North Valley shared project updates: North Valley is evaluating how they can partner with other agencies in their region to address the housing issues. Columbia Basin Hospital is connecting with Confluence Health to strengthen their partnership in transitional care.
- Confluence Health presented their Home-Based Care Transition Pilot plan. The goal is to start implementing this work in Spring 2020.
- TCDI Partners submitted their quarter 4 reports and project plans on January 15. NCACH staff have been reviewing reports for the last 2 weeks of January. A full report will be provided to TCDI partners and the Board at the end of February.

Coalitions for Health Improvement (CHI)

- The Regional CHI Leadership Council met January 15, 2020. Topics discussed included future plans for the CHIs, CHI Community Initiatives Funding, and a recent evaluation finalized by the Center for Community Health and Evaluation.
- Next Meeting: February 13, 2020; (Group met January 15, 2020)
- CHI Community Initiatives Funding update: The NCACH Governing Board approved $99,900 to support 5 Small Projects through the 2019 CHI Community Initiatives funding process. The remaining $350,100 will be allocated to Medium - Very Large projects through the community investment process in February 2020. Read more here
- The NCACH Governing Board also approved an additional $450,000 in funding for 2020 during their last meeting on December 2. More information, including application submission cycles and dates, will be shared as it becomes available.
