They are one of five 2019 MTF Community Service Grant recipients, click above to learn more.

MSP Benefits to Authorization for Payment

 

FROM MSP:

Please be advised that effective January 15, 2020 practitioners who are opted out of MSP (soft opted-out) and wish to receive payment directly for reimbursements for benefits payable to a beneficiary will be required to fill out an “authorization for payment from Medical Services Plan to opted-out practitioners (HLTH 2947) form”.  The form is available on the forms for practitioners/authorization forms page.
 
This form will replace the assignment of medical services plan benefits to opted out practitioner form formerly sent out with and referenced in the “Welcome to MSP” letter for opted-out practitioners.  Practitioners may duplicate this form as needed and each patient must complete the form when the MSP reimbursement will be directed to the practitioner.  The practitioner must keep a completed copy for administrative purposes.  If a dispute regarding payment arises on behalf of any beneficiary the form must be made available to MSP upon request.
