The Area Agency on Aging for Southwest Florida has received a grant to offer the Stanford-designed Chronic Disease Self-Management Program in our area. We will be offering classes soon in Collier County!

We need volunteers to help lead these workshops!
We will be hosting a free four-day training for the Chronic Disease Self-Management Program in Collier County in late March or Early April. (Dates and location TBA).  Contact us before February 28 to reserve your spot in our Collier County training group!

The Chronic Disease Self-Management Program is an evidence-based workshop designed to help older adults and adults with disabilities to make a step-by-step plan to improve their health and their lives.  It is one of several Health & Wellness classes we are able to offer across Southwest Florida.
