Grace Limandjaja investigated skin biopsies of keloids to identify defining keloid characteristics and used cells from these skin biopsies to develop a true-to-life in vitro keloid model, for the purpose of further advancing our understanding of the underlying causes of keloid formation.

Evaluation of bone health within blood or urine is a challenge for a clinician. Bone turnover markers are factors that are released when bone is degraded or formed, and therefore display the actual bone metabolism. P1NP and CTx are the most frequently used markers. However, do these biomarkers have true clinical value in the diagnosis and follow up of patients with bone (related) diseases? Moreover, what are the analytical possibilities and pitfalls of these markers? These questions are addressed in this thesis.

This thesis focuses on scars. It discusses various aspects of scars, including their assessment, pathophysiology and treatment options. Scar evaluation tools are necessary for an evidence-based approach to scar management. A number of rare congenital syndromes are reported to be associated with keloids. The most inconspicuous scar possible was compared to the closure of median sternotomy incisions in children with regard to wound healing and scar formation by comparing subcuticular sutures with Steri-StripTMS. The last chapter is dedicated to the effect that age has on scar formation.

To improve gait, persons with calf muscle weakness can be provided with a dorsal leaf spring ankle foot orthosis (DLS-AFO). These AFOs can store energy during stance and return this energy during push-off, which, in turn, reduces walking energy cost. Simulations indicate that the effect of the DLS-AFO on walking energy cost and gait biomechanics depends on its stiffness and on patient characteristics. Therefore the effect of varying DLS-AFO stiffness is studied on reducing walking energy cost, and improving gait biomechanics and AFO generated power in persons with non-spastic calf muscle weakness, and also whether the optimal AFO stiffness for maximally reducing walking energy cost varies between persons.

The studies enclosed in this thesis focus on two specific types of arthroplasty of the upper extremity: radial head- (RHA) and total elbow arthroplasty (TEA). The thesis is divided into two parts, both aiming to analyze clinical failure mechanisms of elbow arthroplasties so that it gives clinicians more insight, that it may improve the results of elbow arthroplasties, including the treatment of complex elbow trauma. Both parts are introduced by a chapter reporting on the detailed anatomy of the radial head and the proximal ulna using micro-computed tomography (micro-CT) imaging. Part I focusses on RHA with specific attention to indications, diagnosis and management of revision surgery and the radiographic and clinical outcomes thereafter. Part II centers around TEA, focusing on trends and indications for primary and revision surgery of TEA and radiographic and clinical outcomes after revision surgery.
