The 2020 Million Hearts® Hypertension Control Challenge is now open!
Hypertension, or high blood pressure, is common and dangerous but controllable. Yet only about 1 in 4 adults with hypertension have their condition under control. The Hypertension Control Challenge, a key part of the Million Hearts® 2022 initiative, identifies and spotlights health care professionals, practices, and systems that have achieved exemplary control rates in their patients with hypertension. Since 2012, Million Hearts® has recognized 118 Hypertension Control Champions, who together serve more than 15 million adults across 37 states.

You, your teams, and your networks are now challenged to join this stellar group of high performers by submitting your application for the 2020 Million Hearts® Hypertension Control Challenge. The Challenge is open to solo and group practices and health systems that achieve hypertension control rates of at least 80% for a 12-month period. Learn more about rules and eligibility requirements and submit applications. The Challenge is open through April 6, 2020.


Million Hearts® Hospitals and Health Systems Recognition Program
Heart disease, stroke, and other cardiovascular diseases (CVDs) kill more than 800,000 Americans each year, accounting for 1 in every 3 deaths. CVD is the nation’s number one killer among both men and women and the leading cause of health disparities across the population.

Improving outcomes for people with or at risk for CVD is achievable. Million Hearts® is pleased to announce the Million Hearts® Hospitals & Health Systems Recognition Program, a program that recognizes institutions working to systematically improve the cardiovascular health of the population and communities they serve through the priority areas of (1) keeping people healthy, (2) optimizing care, (3) improving outcomes for priority populations, and (4) innovating for health. Eligible applicants include health systems with multiple hospitals, hospitals with and without ambulatory medical practices, and medical practices not affiliated with hospitals.
 
The Million Hearts® Hospitals & Health Systems Recognition Program is now open. Applications received by April 30, 2020, will be among the first to be considered and to be eligible for this designation. Applications will be reviewed and considered on a quarterly basis. New CPSTF recommendation on tailored pharmacy-based adherence interventions for cardiovascular disease prevention
The Community Preventive Services Task Force (CPSTF) recommends tailored pharmacy-based adherence interventions for cardiovascular disease prevention. Evidence shows interventions delivered by pharmacists in community and health system pharmacies increased the proportion of patients who reported taking medications as prescribed.

Tailored pharmacy-based interventions aim to help patients who are at risk for cardiovascular disease take their medications as prescribed. Interventions include assessing barriers to adherence, providing tailored guidance including medication counseling or motivational interviewing and services including medication refill synchronization, and enhanced follow up.

New Diabetes Statistics Report released
The National Diabetes Statistics Report is a periodic publication of the Centers for Disease Control and Prevention (CDC) that provides updated statistics about diabetes in the United States for a scientific audience. It includes information on prevalence and incidence of diabetes, prediabetes, risk factors for complications, acute and long-term complications, deaths, and costs. These data can help focus efforts to prevent and control diabetes across the United States. Below are some quick stats from the report.

Diabetes Fast Facts:

  • Total: 34.2 million people have diabetes (10.5% of the US population).
  • Diagnosed: 26.9 million people, including 26.8 million adults.
  • Undiagnosed: 7.3 million people (21.4% of people with diabetes are undiagnosed).

Prediabetes Fast Facts:

  • Total: 88 million adults aged 18 years or older have prediabetes (34.5% of the adult US population).
  • 65 years or older: 24.2 million adults aged 65 years or older have prediabetes.

March is National Kidney Month
Did you know that high blood pressure is the second leading cause of kidney failure in the U.S.? Chronic kidney disease is a serious condition that increases your chances of having a variety of complications such as stroke, heart attack, kidney failure, and death.

That’s why this March, in honor of National Kidney Month, The National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) is partnering with the National Heart, Lung, and Blood Institute to encourage you to manage your high blood pressure and help protect your kidneys. Online course: Chronic Disease Care within the Community Pharmacy: Maximizing Medication Therapy Management
The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences is offering a no-cost Continuing Professional Education through its online platform entitled Chronic Disease Care within the Community Pharmacy: Maximizing Medication Therapy Management. By the end of the course, participants will be able to:
  • Describe the current reimbursement issues within community pharmacy.
  • Summarize programs aiming to transform community pharmacy.
  • Explain the benefits and limits of medication therapy management.
  • Utilize medication therapy management to improve the management of diabetes mellitus, hypertension, and dyslipidemia.

New Million Hearts® in Municipalities Toolkit released
Use the Million Hearts® in Municipalities Tookit modules individually or as a package to identify or expand evidence-based strategies that are appropriate for the specific needs of your community. The target audiences for this toolkit are public health officials, epidemiologists, health system analysts, and clinical care teams working in municipal public health offices, as well as people working in non-governmental organizations and other local public health organizations.

The current state of diabetes in Colorado
This new 2019 report from CDPHE contains up-to-date information on diabetes burden management needs, comorbidities, costs and more.

Colorado Diabetes Educator Facebook group
This group is intended for accredited or recognized Diabetes Self-Management Education and Support (DSMES) programs, or diabetes educators interested in accrediting their program. This is a place for DSMES providers to network, ask questions of their peers, and exchange DSMES-related information. You can request to join here or by searching for “Colorado Diabetes Educator Network” on Facebook.

Healthy Hearts Calculator
The Healthy Hearts Calculator is an interactive tool that provides estimates of projected health and economic benefits from implementing cardiovascular prevention programs and policies. This tool can be a starting point for state health departments, health insurers, and health care delivery systems to better understand how various interventions can impact the health of their population. Additional features and interventions may be added at a later date.

CDC adult obesity prevalence maps
CDC has released the 2018 Adult Obesity Prevalence Maps for all 50 states, the District of Columbia, Guam and Puerto Rico. Adults with obesity are at an increased risk for many serious health conditions such as heart disease, stroke, type 2 diabetes, some cancers, poorer mental health and more. Obesity costs the United States health care system over $147 billion a year. In addition, research has shown that obesity affects work productivity and military readiness.

Rural telehealth toolkit
The Rural Telehealth Toolkit is designed to help rural communities identify and implement telehealth programs. Developed by the National Opinion Research Center (NORC) on behalf of the Federal Office of Rural Health Policy (FORHP), the toolkit compiles promising approaches and resources to support rural communities seeking to develop and expand telehealth programs. This toolkit presents practical strategies that can help rural communities identify successful telehealth models, address common implementation challenges and prepare for long-term sustainability.

The Community Clinical Linkages Toolkit

The Million Hearts® Collaboration, American Heart Association and other stakeholders have unveiled a new tool to promote community-clinical linkages: The Community Clinical-Linkages Toolkit. The Toolkit organizes an array of resources in a handy grid format to help public health practitioners implement the seven step-by-step strategies that are highlighted in the Community-Clinical Linkages Practitioner’s Guide under the LINKAGE framework.

This microsite includes Million Hearts® evidence-based protocols, action guides, and other quality improvement tools to help clinicians and other health professionals reduce their patients’ risk for a heart attack or stroke. American Diabetes Association provider videos

The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length.

Clinical pharmacy resource toolkit

As part of the patient’s health care team, pharmacists can improve disease outcomes by improving medication adherence and providing patient planning, education and counseling. The Community Preventive Services Task Force found strong evidence that team-based care can improve blood pressure control when a pharmacist is included on the team. A new toolkit from The University of Colorado provides sample agreements and resources helpful in integrating clinical pharmacists into the care team. Click for more information.

The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, with support from the Colorado Department of Public Health and Environment, created a series of online educational modules that highlight the role of pharmacists in the prevention and treatment of cardiovascular disease. To view the modules:
  1. Navigate to ipharmCE.UCDenver.edu (or http://ipharmce.ucdenver.edu/student/exam). You will need to create a profile if you do not have one. To create a profile, enter your email under “New Account Sign-up,” and follow the instructions on the screen.
  2. Once logged in, click “View All Programs” under Home Study Activities on the left side of your screen.
