New pharmacy referral service to help patients avoid hospital readmission

A NHS Discharge Medicines Service has been launched by the Department of Health and Social Care to give patients extra guidance around new prescribed medicines after being discharged from hospital.

Patients who could benefit from additional support after hospital discharge will be digitally referred to community pharmacists, to ease pressures on A&E and general practice.

The AHSN Network has led the way with this work, by working on Transfer of Care Around Medicines (TCAM) since 2018 . TCAM has been adopted by 58 hospital trusts in England and has the potential to save £28.8 million in 2020, based on a reduction in length of stay of 113,406 days and 2,007 fewer readmissions.

What do people prioritise when returning to work after major musculoskeletal injury?

The University of Southampton, in partnership with the Royal College of Occupational Therapists, is running a series of workshops for the public and patients to find out what is important to return to work after major trauma injury.

They are looking for adult volunteers for these workshops to see what people (and family/friends) prioritise when returning to work after major musculoskeletal trauma, especially those who have experienced, or supported a loved one recovering from major musculoskeletal injury in the last 24 months.

You will work alongside healthcare professionals and academics from the University of Southampton during a 1-hour workshop. The workshop will involve you identifying and ranking priorities for returning to work with other public and patients. The workshop will involve one visit to the University of Southampton or Southampton General Hospital.

You will be compensated £10 (plus travel expenses) for your valuable time. The project is funded by the Royal College of Occupational Therapy and sponsored by the University of Southampton. After the workshop, should you like to take part in further workshops, please speak to the investigator.

If you are interested in taking part, please email James Gavin or call: 07825 884983.

Recap on the fundamentals of genomics
The Genomics Education Programme has launched two new courses as part of its Genomics 101 series. From Gene to Protein has been created to give health professionals an understanding of proteins, how they are synthesised and how the information in our genes is used to build them. The second course; Dominant Recessive and Beyond has been created to help health professionals understand the common modes of inheritance of genetic conditions and to be able to recognise them based on the pattern of affected people in a family history. The courses in this series each focus on a key aspects of genomics, whether the underlying science or the clinical application. There are currently six courses with a further two to be added later this year, each lasting around 30 minutes in duration. Please share across your trusts, as these courses provide a good grounding in, or recap on, the fundamentals of genomics.

New Genomic Variant Interpretation Course
The University of Manchester have launched a new continuing professional development (CPD) course, funded by the HEE, which provides an introduction to the tools and procedures used in the interpretation of genetic variants in the clinical setting. The next courses will take place on the 12th and 31st March 2020 and will give the learners confidence to participate fully in MDT discussions on variant interpretation and to understand the laboratory variant report. Over 3,000 strokes prevented and 800 lives saved through national drive to detect and treat irregular heart rhythm

Recently released figures reveal that a programme rolled out across the NHS to reduce strokes related to an irregular heart rhythm prevented 3,165 strokes and 791 lives last year (2018/19).

AHSNs have played a key role in this. Pulse checks for over 65s, mobile ECG devices for GP surgeries and pharmacies, and new ‘virtual clinics’ involving specialists working with GPs to advise on the best treatment for people with the condition, were amongst the varied activities undertaken as part of this life saving work.
