LIFEPlan CCO NY continues to monitor and assess the evolving COVID-19 (Coronavirus) pandemic. We will continue to keep our provider partners informed of the work that we are doing with OPWDD, DOH (Health Home Department), and our CCO partners.
This document describes how the CCO's are identifying and prioritizing needs for individuals with high medical, support or resource needs.
The CCOs have collaboratively developed this protocol with OPWDD and DOH to screen our members during this public health crisis.
Care Managers will beginning screening members who live in the community, and later will apply this tool to all members of the CCO.
