Please find a link below to the form which Essential Key Workers must complete to request places at the school-based childcare provision which is being put in place at Combe Martin Primary School from Monday 23rd March. The letter attached sets out who Essential Key Workers are and which children this can apply to.
Please note that places at the provision are not guarenteed until a confirmation email is received.
Please return this form if you think you are eligible for childcare. We will be sending information on how to book in to parents who fulfil the criteria.
The deadline for return is 6 pm on Saturday 21st March, however, we would be grateful if you could respond as soon as possible.
