Strategic Planning and Stakeholder Engagement Sessions for Colorado’s Chronic Disease and Tobacco Programs

UPDATE: Stakeholder engagement sessions have been postponed

Stakeholder engagement sessions (originally planned for March/April 2020) have been postponed so that CDPHE, local public health agencies, health care, and other community partners are able to dedicate their staff and resources to respond to the Coronavirus Disease 19 (COVID-19) and practice social distancing.

An updated timeline for strategic planning and the stakeholder engagement sessions will be shared at Review Committee meetings, with Tobacco and CCPD grantees, and on Review Committee web pages.

Strategic Planning
Together with its partners, CDPHE’s Health Promotion and Chronic Disease Prevention Branch (HPCDPB) implements strategies that advance health equity and the health of all Coloradans through the promotion of wellness, healthy living, and chronic disease prevention and management. Colorado’s statewide Tobacco and Chronic Disease programs are currently engaging in strategic planning efforts to set the direction for the CDPHE and its partners’ future work on these issues. We want your input.

CDPHE’s chronic disease and tobacco programs contracted with JSI Research and Training Inc. (JSI) to assist with the strategic planning and stakeholder engagement processes for the state’s chronic disease and tobacco programs.

The resulting products of this planning process will include an update to the chronic disease state plan (2021-2030) and the development of a new tobacco strategic plan (2021-2030). The new strategic plans will inform the next directions of the Tobacco and the CCPD Grant Programs. The strongest plans for Colorado must reflect the insights and ideas of our partners and the lessons learned from the evaluation of currently funded projects. The new plans are likely to include Public Health 3.0 concepts that include cross-agency collaborations and address chronic disease, tobacco, and determinants of health. We encourage you to participate in the stakeholder engagement process ahead.

Community health workers: Evidence of their effectiveness

The Association of State and Territorial Health Officials and the National Association of Community Health Workers recently published a summary of the research examining community health workers over the past 50 years. This research has focused on assessing their effectiveness in improving health outcomes, reducing health care costs, and bridging the gap in health disparities. This work is supported through the Health Resources and Services Administration’s cooperative agreement with the National Organizations of State and Local Officials. Read the summary.


Root Cause Coalition: Status of Health Equity Report
This report discusses three actionable strategies to achieve health equity, including:

  1. Scale innovative solutions to drive a new and sustainable model of care that improves health outcomes as it ensures health equity.
  2. Align communities and advance authentic collaboration to address the root causes of health inequities.
  3. Engage and learn from communities most affected by inequities of health and social conditions.

The recording of ADCES' live webinar on telehealth, "Understand the new COVID-19 telehealth regulations just released by CMS and how you can get up to speed, fast!" is now available online to view here: DiabetesEducator.org/COVID19. ECHO for primary care
COVID-19 Just-in-Time ECHO for Primary Care is a drop-in series offered three days a week starting on Friday, March 27. An interdisciplinary panel of experts will be on hand to answer your questions and to share up-to-the-minute information about the spread of the virus in Colorado, where and when patients should be sent for testing, medication management, chronic care needs, telehealth, and many other topics. Webinar: A population health strategy to reduce heart disease risk

New Million Hearts® in Municipalities Toolkit released
Use the Million Hearts® in Municipalities Tookit modules individually or as a package to identify or expand evidence-based strategies that are appropriate for the specific needs of your community. The target audiences for this toolkit are public health officials, epidemiologists, health system analysts, and clinical care teams working in municipal public health offices, as well as people working in non-governmental organizations and other local public health organizations.

The current state of diabetes in Colorado
This new 2019 report from CDPHE contains up-to-date information on diabetes burden management needs, comorbidities, costs and more.

Colorado Diabetes Educator Facebook group
This group is intended for accredited or recognized Diabetes Self-Management Education and Support (DSMES) programs, or diabetes educators interested in accrediting their program. This is a place for DSMES providers to network, ask questions of their peers, and exchange DSMES-related information. You can request to join here or by searching for “Colorado Diabetes Educator Network” on Facebook.

Healthy Hearts Calculator
The Healthy Hearts Calculator is an interactive tool that provides estimates of projected health and economic benefits from implementing cardiovascular prevention programs and policies. This tool can be a starting point for state health departments, health insurers, and health care delivery systems to better understand how various interventions can impact the health of their population. Additional features and interventions may be added at a later date.

CDC adult obesity prevalence maps
CDC has released the 2018 Adult Obesity Prevalence Maps for all 50 states, the District of Columbia, Guam and Puerto Rico. Adults with obesity are at an increased risk for many serious health conditions such as heart disease, stroke, type 2 diabetes, some cancers, poorer mental health and more. Obesity costs the United States health care system over $147 billion a year. In addition, research has shown that obesity affects work productivity and military readiness.

Rural telehealth toolkit
The Rural Telehealth Toolkit is designed to help rural communities identify and implement telehealth programs. Developed by the National Opinion Research Center (NORC) on behalf of the Federal Office of Rural Health Policy (FORHP), the toolkit compiles promising approaches and resources to support rural communities seeking to develop and expand telehealth programs. This toolkit presents practical strategies that can help rural communities identify successful telehealth models, address common implementation challenges and prepare for long-term sustainability.

The Community Clinical Linkages Toolkit

The Million Hearts® Collaboration, American Heart Association and other stakeholders have unveiled a new tool to promote community-clinical linkages: The Community Clinical-Linkages Toolkit. The Toolkit organizes an array of resources in a handy grid format to help public health practitioners implement the seven step-by-step strategies that are highlighted in the Community-Clinical Linkages Practitioner’s Guide under the LINKAGE framework.

Million Hearts® for Clinicians Microsite

This microsite includes Million Hearts® evidence-based protocols, action guides, and other quality improvement tools to help clinicians and other health professionals reduce their patients’ risk for a heart attack or stroke. NIMHD Language Access Portal

The National Institute on Minority Health and Health Disparities (NIMHD) has launched a new resource for our stakeholders who work with health disparity populations with limited English proficiency: the Language Access Portal (LAP).

Health information to help people with diabetes better manage their health

The health information here is informed by National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) research, reviewed by doctors, and provided to help you understand the diseases and conditions you or your loved ones may face.

American Diabetes Association provider videos

The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length.

Clinical pharmacy resource toolkit

As part of the patient’s health care team, pharmacists can improve disease outcomes by improving medication adherence and providing patient planning, education and counseling. The Community Preventive Services Task Force found strong evidence that team-based care can improve blood pressure control when a pharmacist is included on the team. A new toolkit from The University of Colorado provides sample agreements and resources helpful in integrating clinical pharmacists into the care team. Click for more information.

Updates in Pharmacist Cardiovascular Care online study modules

The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, with support from the Colorado Department of Public Health and Environment, created a series of online educational modules that highlight the role of pharmacists in the prevention and treatment of cardiovascular disease. To view the modules:
  1. Navigate to ipharmCE.UCDenver.edu (or http://ipharmce.ucdenver.edu/student/exam). You will need to create a profile if you do not have one. To create a profile, enter your email under “New Account Sign-up,” and follow the instructions on the screen.
  2. Once logged in, click “View All Programs” under Home Study Activities on the left side of your screen.
  3. Under the group Updates in Pharmacist Cardiovascular Care, click on the individual presentation titles.
