Glutathione: IV, Nebulized, and Oral

N-acetyl Cysteine (NAC) and Coronavirus

There have been several articles written and news media touting a ‘drug’ that could save patients from needing intubation while fighting the coronavirus. The claims indicated that a drug, N-acetylcysteine (NAC) can prevent the virus from entering the lungs.  While this is most likely true, you first must know that NAC is not a drug. It is a naturally occurring amino acid and has been used for nearly a century.  NAC is used conventionally for Tylenol poisoning and by functional doctors for many diseases, but especially those of toxicity and diseases and infections that affect the lungs and upper respiratory tract.

N-acetyl cysteine is directly a mucolytic, which means it breaks mucous and decreases viscosity of sputum and cleans the lungs.  It also is used to promote glutathione production.  The production of glutathione is where the majority of the NAC benefits lie.

Let’s talk about why this is so. But first let’s go through some basics.

What is glutathione?

Glutathione Is our body’s most important antioxidant. It is made of 3 amino acids: cysteine, glycine and glutamate.  Cysteine is the backbone of glutathione and typically is the rate limiting amino acid, meaning its usually the one we are lowest in.  Glutathione is found in nearly every organ but especially those of detoxification – liver, kidneys, lungs, GI tract and skin.

Why do we need glutathione?

Glutathione helps our body eliminate heavy metals (such as arsenic and mercury), chemicals (such as formaldehyde, acetaminophen, and chlorine), environmental toxins (mycotoxins from mold) and our own internal toxins (such as hydrogen peroxide).  Glutathione also plays a role in supporting immune function, DNA replication and assisting apoptosis (normal programmed cell death).

Where do we get glutathione?
  1. Diet.  Glutathione is directly present in our diet.  Those with the best diets get about 150 mg per day and those with the worst diets get as little as 3 or less milligrams per day.  Glutathione is present in fruits and vegetables, nuts, poultry and fish.  All glutathione is lost during processing of foods (exception of fresh-frozen foods) as well as the curing and canning of our foods. Cereals and grains, dairy products and sweeteners contain no glutathione.
  2. Cellular production.   The amino acids, minerals and vitamins in our diet all help our cells to produce their own glutathione. We need plenty of sulfur amino acids, vitamin C, vitamin E, B vitamins and minerals, such as selenium, magnesium and zinc.  If any of these are lacking, our glutathione production will decline.  Also, many of us carry genetic abnormalities that can slow production and increase risk of disease.
What can cause low glutathione:
  1. Low intake
  2. Chemical exposures.
  3. Aging
  4. Genetic susceptibility - enzyme defects

When should I be concerned about my glutathione levels:
  1. Age. Glutathione levels naturally decrease as we age
  2. Long history of a Standard American Diet (SAD) – high intake of processed foods.   This is due to lack of nutrients as well as other indirect exposures such as acrylamides. We must also be aware that many chemical additives in our foods and chemicals through the cooking will remove glutathione.  The potent neurotoxicant acrylamide is one of these chemicals.  Acrylamides are high in high starch foods that have been cooked in high heat – French fries, donuts, potato chips, etc.  It is not listed on a label because it was produced in the cooking and processing of the food – this is why avoidance of processed foods is of very high importance – The more man made, or man manipulated the worse the food will be for our bodies.  Without proper glutathione ingestion and production, disease will eventually manifest.
  3. Known environmental and chemical exposures – heavy metals, mold, welding, pesticides, cigarettes, solvents, etc.
  4. Inflammatory lung diseases – asthma, pulmonary fibrosis, emphysema, chronic bronchitis etc.
  5. Mitochondrial and neurological diseases – Parkinson’s disease, cancer, Alzheimer’s, migraines
  6. Chronic infections – hepatitis, HIV, etc.
  7. Cancer
  8. Autoimmune diseases– Multiple sclerosis, lupus, RA, inflammatory bowel diseases
How can I improve my Glutathione levels?
  1. N-acetyl cysteine (NAC) Supplementation.  Many believe by taking N-acetyl Cysteine (NAC) their bodies will automatically produce more glutathione.  While this is true, and often it will be true especially in very healthy individuals, there are many other genes and cofactors involved in order to produce glutathione. The mistake of thinking NAC automatically increases glutathione is kind of similar to saying since cake is made of flour, if I get more flour, I will automatically have more cake.  And we all know that is not true.  Without enough extra butter, eggs, salt, etc. we may not be able to make more cake with more flour alone. So, if we supplement NAC, we also need to assure we have good levels of vitamin C, vitamin E, B vitamins and minerals, such as selenium, magnesium and zinc.  If we do not have enough of these, NAC can produce reactive nitrogen and sulfites.  The intake of riboflavin (B2) and molybdenum will aid in the breakdown of sulfites if they are produced.
  2. Glutathione oral Supplementation. S-acetyl glutathione (Vis Intracellular Glutathione) is a supplemental form that will increase tissue levels throughout the body.  Avoid capsulated reduced glutathione as this supplement will not pass the digestive tract without being broken down by stomach acid and digestive enzymes.  Direct oral supplementation needs to start off low and gradually increase.  Many people need to take extra minerals and Active B vitamins to properly process exogenous glutathione supplementation.
  3. Intravenous Glutathione.  This form is utilized more in those who have known heavy metal and other environmental exposures to aid in detoxification.  We also will use it during times of acute illness to aid the immune system – especially in those with respiratory disorders. Regular infusions may produce significant improvement in many chronic inflammatory diseases.
  1. Nebulized Glutathione – A nebulizer is used to turn liquid glutathione into a fine mist to be inhaled through a face mask or mouthpiece. This is the best way to directly replenish glutathione in the lungs and respiratory pathways.  This form of glutathione is especially good for those who already have known conditions of the respiratory tract – pulmonary fibrosis, asthma, emphysema, lung cancer – as well as those who have had chronic aerosolized chemical exposures – cigarette smokers, wood workers, welders, chemical sprayers, etc.   Typically for these disorders we recommend regular use to keep replenishing the cells and to eventually help increase their own production. We will often use this therapy in conjunction with oral and IV glutathione.  Nebulized glutathione can also be successfully utilized in acute viral infections to aid in mucous removal and healing of the alveolar sacs of the lungs.

Are there any side effects to supplementing Glutathione and/or NAC?

In generally healthy individuals, supplementation will create no side effects.  Those who have significant disease or where glutathione production has been compromised, we recommend that they start with a low dose and increase over time.  We also will recommend supplementation of Active Minerals and Active B Vitamins. This is one reason why we generally do a vitamin C IV with B vitamins and Minerals prior to Glutathione IVs and typically include Minerals and B vitamins in a supplemental regimen.  If you develop a headache, irritability, insomnia, foul smelling gas, stool or urine, this is a sign you need to back down on your dosage and increase the Minerals and B vitamins, especially selenium, molybdenum and riboflavin (B2). Long term high dose NAC can also cause extreme dryness in some individuals as well.

If I am to supplement, should I use NAC or glutathione?

In majority of individuals both can successfully be utilized.  It is best to talk with one of our VIS Clinic providers to determine what may be best for you.
