In the event that you or your family member becomes hospitalized, or need to visit to the Emergency Room during the COVID-19 public health emergency,  the Self-Advocacy Association of New York State (SANYS) prepared some helpful communication materials.   

The SANYS documents contain the following information for your review and use:

• An overview and summary document, Help For Going To The Hospital.  Click to download.
• A script for you or the support person to complete, advocacy information on your rights and emergency phone numbers for the Care Coordination Organizations/Health Homes.  Click to download.

We suggest you create an emergency file with this information and any other documents relevant to your care.  While we hope that you will not need the hospitalization documents, we want you to be prepared.
