Chiropractic Care in Lockdown

Dear TBCers

We are in an unprecedented set of circumstances and in many ways, chiropractic wasn't set up to provide care in these conditions.
Our registration board did not have any relevant guidelines in place, ACC didn't have any non-face-to-face chiropractic services in their system, practices didn't stock personal protective equipment (PPE) for chiropractors beyond gloves, and our cleaning protocols didn't include hospital grade sprays.
It has been a time of learning and developing, growth, meeting and overcoming challenges, sourcing products we haven't had to source before and drafting and writing additional 'best practice' guidelines! On top of this our chiropractors have been feeling a bit out of sorts at not being able to provide you the care that you (and they) would like, team members have been missing each other and all of you, and we have all been dealing with the widespread changes in our lives and the uncertainty that we are currently living with.

So, to the Good News
We are pleased to report that we have got through many of the items in the list above and can now offer some certainty about the services that we are able to provide in lockdown - both Levels Three and Four.
Telehealth
Corrian and Hamish are available for formal Telehealth consults.

You may wish to discuss the things that you can be doing at home to support your health and wellbeing to address a niggle or an ache. Maybe you want to learn some rehabilitation or wellbeing stretching and strengthening exercises that are particularly relevant to you to support YOUR spine. Or perhaps you would like to discuss your 'work from home' set up and check that it is best for you. However you wish to draw on their broad chiropractic knowledge which encompasses a lot more that the 'hands on' component of care, Hamish and Corrian are happy to help. They would be pleased to support you in preventing that niggle or ache from developing into something pretty painful.

Telehealth consults may be by telephone, video services, messenger calls, Zoom etc but must be conducted in a way that protects your privacy. As mentioned by Corrian in our article in the previous edition of our updates, we have been really pleased at how successful these calls to date have been for you.

The charge for a Telehealth appointment is $30. Our preference is for this to be paid online. If you don't have this facility then you will be able to pay it once we move to Alert Level Two and you are next in the practice.

To book an appointment, please leave a message on the TBC answer phone on 03 5444 554. Our CA team will be clearing messages at 8.30am, 12.30pm and 4pm Monday-Friday and 10am on Saturdays. They will then call you back. Bearing in mind that we employ a 'best practice' philosophy, all appointments will be prefaced by the completion of a short COVID-19 risk factor questionnaire.

Corrian is available for Telehealth appointments from 9am-1pm Monday-Friday and 10am-12 noon on Saturdays.

Hamish is available for Telehealth appointments from 11am-4pm Monday-Friday and 10am-12 noon on Saturdays.

As always, if your situation is urgent, please call them directly.
 
Telehealth and ACC
We are really pleased that ACC has now made funding available for chiropractors to provide ACC Telehealth care.
This service is available for you if you were receiving regular care on an ACC claim prior to lockdown, or for a new ACC claim.
There is no charge to you for a Telehealth appointment. Set up this appointment by ringing the practice landline as above.

Emergency Care
Quite a number of you have been contacting your chiropractor regarding this care and we have been working through the protocols and sourcing the required PPE gear so that we can serve those of you who are deemed to need emergency care.

Before your case qualifies for emergency care, your chiropractor is obliged to complete a Telehealth appointment. Chiropractors have very strict guidelines about who they are able to provide emergency care to, so any follow-up booking conversation will include 'triage' questions to help them determine whether your case meets the criteria.

All emergency care visits are required to be logged with the chiropractic registration board for the purposes of contact tracing should that be required.
 
We will be closely following all Ministry of Health guidelines around the provision of face-to-face care.
 
Our regular visit fees apply for emergency care and the Telehealth fee to set up this visit may also apply.

New Patients
A number of you are referring friends and family member to chiropractic care - thank you for thinking of us. Yes we are able to provide both the above services to new folk. We will do our best to keep the costs of this to as close to those listed above, but if there is considerable time/complexity involved, there may be a slightly higher rate charged.

NZ Chiropractors Available Online During Lockdown and Level 3
 
New Zealand Chiropractors are able to provide online consultations to Kiwis and give advice on how to keep their spines mobile and healthy during the COVID-19 lockdown and beyond.
 
Running, cycling, online workouts and DIY are hugely popular at the moment as New Zealanders adjust to isolation at home and find ways to keep fit and busy. But ACC and other parts of the health service are concerned that these sudden bouts of activity will lead to a spike in injuries putting undue pressure on the system.
 
As Dr Cassandra Fairest, chiropractor and Vice President of the New Zealand Chiropractors’ Association explains: `Normally, you’d book an appointment to visit your health care practitioner, but during COVID-19, unfortunately you can’t just walk into a practice. Thankfully, Telehealth is a way to see your chiropractor without leaving your home. It’s injury-prevention and professional health advice in your living room.’
 
Dr Fairest says Telehealth has been on the horizon for years, but COVID-19 has accelerated the process of getting the practice into clinics and the public familiar with it.
 
She adds: ‘Although we’re not able to use our hands during a Telehealth session, chiropractors are highly trained in all manners of physical assessment.  Even with simple visual exams, a detailed case history and clinical reasoning, there’s a great deal that can be done to advise, help and reassure in a Telehealth session. Via video links, we’re able to assess overall movement and posture for example. We can also give self-care advice such as tailored stretches or exercises, to help improve areas of tension and increase global function and posture. There’s often a number of simple and effective things you can do to help stay healthy and either get yourself back on track or at least more comfortable, until you’re able to come in for a face-to-face visit as the Alert Levels reduce and restrictions ease.’
 
‘It’s important to note that we can also assess the likelihood of whether something more problematic is going on.’  Unlike using Google to self-diagnose your problem and getting a dozen confusing or conflicting results, your chiropractor can usually accurately assess your situation. Chiropractors understand that poor movement and function of the spine can lead to overall dysfunction as well as an accumulation of health issues. Maintaining good posture, neurological integrity and movement are essential to wellness. This is particularly important when our normal schedules have been disrupted and patterns of physical, mental and emotional stresses accumulate.
 
For a Telehealth session, your NZCA chiropractor will provide a link to a secure video chat. Your chiropractor will talk with you about what’s going on, assess how you move and suggest home care for your injury. Most chiropractors during Lockdown and under Level 3 have remote reception services up and running, or will be able to take your call directly.  Dr Fairest notes that each practice may vary with the particular systems they use and potential charges, if any, but recommends calling your local NZCA member chiropractor to discuss options.
 
‘We are encouraging people to take steps to be kind to their spines and nervous systems, to promote ongoing wellness and prevent long term dysfunction and injury in order to reduce the likelihood of problems post-Lockdown.’
 
NZ Chiropractic Board Telehealth Guidance and Information:
The Telehealth Standard definition - “Telehealth is the use of information and video conferencing technologies, to deliver health services and/or transmit health information to a patient when the clinician is in a different location.”
 
All Chiropractors providing Telehealth consultations need to meet the following criteria:

 
  • be registered and hold a current Annual Practicing Certificate from the Regulatory Authority
  • ensure the use of Telehealth is clinically appropriate
  • complete a risk assessment and determine if Telehealth is a safe method of delivery; your risk assessment may need to be continually reviewed
  • provide an appropriate standard of service to clients based on their clinical need
  • obtain the client’s consent to provide services via Telehealth.
