LVHN COVID-19 Assess and Test–Sherman Court Moves to Larger Location  

In order to provide care to a greater number of people, LVHN COVID-19 Assess and Test–Sherman Court in Hazleton is moving to larger location in the same building.

The new site is called LVHN COVID-19 Assess and Test–Vine Street and is located at 149 N. Vine Street, Hazleton. This office was previously used by LVPG Family Medicine–Vine Street. LVPG Family Medicine–Vine Street is temporarily relocating to 140 N. Sherman Court, Hazleton, where it will share space with other temporarily relocated practices: LVPG Family Medicine–Sugarloaf, LVPG Family Medicine–Hometown, LVPG Family Medicine–Mountain Top and LVPG Family Medicine–Brookhill.

LVHN COVID-19 Assess and Test–Vine Street will be open Monday-Friday, 8 a.m.-5 p.m. and Saturday-Sunday, 9 a.m.-3 p.m. People will be referred to this location after pre-test screening determines that they should have an assessment and possible test. Pre-test screening is free and can be done by phone at 888-402-LVHN (5846) or through MyLVHN via E-Visits, COVID-19 Video Screenings or an LVHN Video Visit.
