Parents/Guardians - What is Your Experience with Your Child's Managed Care Organization?  Please Complete This Survey!

Dear Parent or Guardian:

The Imagine Different Coalition is conducting a survey to learn about your experience with your child’s Medicaid health insurance plan. 

If you live in Pennsylvania and your child with disabilities receives Medicaid / Medical Assistance, it is likely to be managed by a Physical Health Managed Care Organization (PH-MCO).  These health plans authorize and pay for children's health-related services in our state.

Your responses are important to us.  They will NOT be shared in any way that identifies you and will have NO effect on the services and supports your child and family receive.  The survey does not request your contact information.  The results will help to identify ways that Pennsylvania can better serve children with disabilities and their families.  You can take the survey at: Parent Survey 2020.  Please respond by August 8th, 2020.
