Thank you for participating in Ottawa New Horizons Band's Fall Session. We'd really like to receive your feedback about this session, what you thought about the music you played, the clinics, band leaders, the concert, what you really liked and where we could improve.

The survey will be available for you to use until December 30, 2018, but please complete it as soon as possible while the session and the concert are still fresh in your mind. Your individual responses will be confidential and only aggregate data will be provided to the board, band leaders and Advisory Committee.
