Managing Medicines in the Community  
A collaborative project across health and social care is aiming to improve the quality of medicines support for people with care and support needs living at home. A campaign is being developed by NICE, ADASS, LGA, NHS England, CQC, Skills for Care, UKHCA, VODG, Think Local Act Personal Partnership, the Royal Pharmaceutical Society and others.  We are aiming to encourage key partners within health and social care to play their role in supporting good practice, and do this by promoting key messages from the NICE guideline and quality standard ‘Managing medicines for people receiving social care in the community’ (NG67 & QS171).  Contacts : Liz Greer (LGA) liz.greer@local.gov.uk and Paul Morgan (ADASS) paul.morgan@surreycc.gov.uk

Reimagining Primary Mental Health Care
This report highlights 8 local initiatives that are providing different approaches to meeting people’s mental health needs in primary care are the focus of a new report by the Centre for Mental Health and University of Birmingham.  Read the report

The LGA are currently in the process of shaping their digital care and health plans for 2019/20. Their work will continue to be reflective and supportive of councils priorities.

Social Care Programme Update

The Social Care Programme in NHS Digital is supporting a number of programmes across the social care Sector.  The programme is supporting the following: 
  • Digitising the Discharge Process: The project is working with councils and social care system suppliers to digitise the assessment and discharge notification flow of information.
  • Social Care Digital Innovation Programme: Working with the LGA this programme is currently funding nine councils to support innovation in social care.
  • Demonstrator Programme: This programme is supporting councils and care providers in both the use of data analytics and support information flows from and into social care. 
  • Care Provider Engagement: NHS Digital has recently announced a new sector led offer for care providers. The Care Provider Alliance will work to understand current levels of digital maturity within the social care sector and assist providers with the adoption of new technologies.

Capacity Tracker- mobilising across the Midlands, South West and South East 

A website to improve patient flow and minimise delayed transfers of care, is being mobilised across the Midlands, South East and South West areas.  Capacity Tracker was developed by NHS NECS, NHS England (North), Local Authorities and care home managers. When the decision has been made that a care home is needed it makes the process of finding a placement easy and it’s free for care homes to use. The Capacity Tracker has been successfully deployed in 37 health and social care systems across North England, Berkshire and Devon. Mobilisation across the three regions has already started and the ambition is to maximise care home uptake as soon as possible, to help alleviate winter pressure.
