Project Updates

New Reimbursement Model
The ONE Rx team is pleased to announce a new reimbursement model for participating pharmacy.  The new model is a fixed $20 reimbursement for every ONE Rx screening completed. This change is effective immediately.  In addition, retrospective reimbursement will be applied to all ONE Rx screenings that have occurred to date. In order to be eligible for this benefit, you need to have 75% of your pharmacists trained in ONE Rx, enroll your pharmacy as a participating ONE Rx provider, and provide access to naloxone and medication take-back in your pharmacy. 

The new model replaces the incentive payments given to pharmacies as part of the ONE Rx Pharmacy Recognition Program.  The recognition program however will continue with certificates and publicity being given to pharmacies meeting certain thresholds.  

ONE Rx in the News 

Wednesday February 6, 2019, is Public Health Day at the Legislature. Oliver Frenzel, PharmD, of the ONE Rx team, will be presenting a poster titled "The Opioid Epidemic in North Dakota: Describing the Pharmacist's Role and Intervention Opportunities." We are hoping to share the story of pharmacy contributions to the opioid crisis with a large number of North Dakota Legislators that day.

 

Data Snapshot


The graphic above represents ONE Rx results through January 21.  These measures show the impact being made across the state.  

Another data measure identifies possible area for improvement.  The prescriber has been contacted in less than 1% of the screenings, which seems low considering 28% of screenings are at risk of overdose and almost 5% being identified at risk for opioid use disorder.  The ONE Rx team understands this can be a difficult conversation to have with the prescriber.  Our next coaching call with provide guidance on how to have these conversations.  See below for additional details.  

Tips From the ONE Rxperts

Pharmacists at pharmacies meeting recognition level status are being dubbed ONE Rxperts (thank you to Michael Dulitz from Grand Forks Public Health for creating the term).  These pharmacists are examples of how ONE Rx can be incorporated into your practice and workflow.  Below are a few of the tips they have shared in their conversations with the ONE Rx team and on our coaching calls.  

-There is never a perfect day to start something new.  You just need to do it.  Pick a day to start and be committed to the process."

- If a patient is upset when asked to complete the screening, explain to them that every patient receiving an opioid is being asked to do so.  They are not being singled out.  The pharmacy is doing this in the name of patient safety.   Most patients agree to complete the screening when explained to them this way.  They are also free to decline to participate and still have their prescription filled.  

REDCap Tip - Use the Code!

Want to make the ONE Rx process more time efficient?  Have the patient complete the ONE Rx Patient Intake Form electronically!  Pharmacies that have patients complete the intake form using a tablet provided by the pharmacy or by having patients scan the QR code (example pictured) using their handheld device report a faster, more convenient process.  Pharmacy staff saves time as they do not have to manually enter the results into REDCap and the ORT is automatically scored by REDCap.  

Each pharmacy has its own specific QR code.  You should have received this with your welcome packet.  Please contact Jayme Steig if you need assistance with this process.

Frequently Asked Questions

How should I respond to a patient who says this survey is "way out of line"?
The preferred method is to gauge the patient and then choose one of the two responses below:
  • In light of new understanding about these medications, we've learned that it is good to take an extra step to ensure your safety.
  • This helps us to know how we can best advise you about your risks and prevent problems from cropping up. 
Other alternatives for use that depend on your relationship with the patient are:
  • We've changed our processes for everyone who is getting pain medication.
  • We're trying to increase our ability to watch out for our patient's safety.
  • Everyone asks you to fill out a form these days: the dentist, the beautician, the massage therapist... the pharmacist is getting in on the act too.
  • We probably should have been doing this all along.

Gold Level – 75% of pharmacists completed ONE Rx training, pharmacy has a medication take-back program, 100 patient screenings completed
