Addressing medication complexity through community-based strategies
With 29 percent of Americans taking five or more medications daily, and 119,000 deaths annually occurring from prescription medication-related issues, it is important to look at medication complexity — a little-discussed topic. This infographic illustrates the issue and outlines community-based approaches to address it, such as developing risk algorithms to identify at-risk patients and simplifying medication use through comprehensive medication management.



Pharmacists and telemonitoring
Implementing a team-based care approach that uses home telemonitoring, pharmacist-led medication therapy management, and patient care support can improve blood pressure control, medication adherence, and patient-pharmacist relationships. High blood pressure facts from the CDC
  • Having high blood pressure puts you at risk for heart disease and stroke, which are leading causes of death in the United States.
  • About 75 million American adults (32%) have high blood pressure - that’s 1 in every 3 adults.
  • About 1 in 3 American adults has prehypertension, blood pressure numbers that are higher than normal—but not yet in the high blood pressure range.
  • Only about half (54%) of people with high blood pressure have their condition under control.

Community Preventive Services Task Force meeting: February 13-14, 2019
You are invited to attend the upcoming Community Preventive Services Task Force (CPSTF) meeting on February 13-14, 2019. Register to attend the meeting in person or via live stream.

During this meeting, the CPSTF will discuss Community Guide effectiveness methods and cover the following topics:

  • Cancer prevention and control.
  • Cardiovascular disease prevention.
  • Health equity.
  • Mental health.
  • Obesity.
  • Physical activity.
  • Reproductive health.

The CPSTF meets three times per year in Atlanta, Georgia. CPSTF members, Liaison representatives, and Community Guide staff attend the meetings, which are open to the public.

2019 MT STEMI and Rocky Mountain Stroke Conferences May 2 & 3 in Whitefish, Montana
The Cardiac Ready Communities program of the Montana Department of Public Health and Human Services will be sponsoring the 2019 MT STEMI Conference at The Lodge at Whitefish Lake on May 2. The one-day event will focus on the recognition and treatment of STEMI cases from EMS through the PCI Center.

On May 3, the Montana Stroke Workgroup is hosting the second annual Rocky Mountain Stroke Conference at The Lodge at Whitefish Lake. Providers from around the state of Montana, as well as two world renowned stroke experts, will come together to share their expertise in ischemic and hemorrhagic stroke, telestroke, stroke program development, prevention and rehabilitation.

The Community Clinical Linkages Toolkit

The Million Hearts® Collaboration, American Heart Association and other stakeholders have unveiled a new tool to promote community-clinical linkages: The Community Clinical-Linkages Toolkit. The Toolkit organizes an array of resources in a handy grid format to help public health practitioners implement the seven step-by-step strategies that are highlighted in the Community-Clinical Linkages Practitioner’s Guide under the LINKAGE framework.

The VISION tool provides data for chronic disease and behavioral health measures in Colorado. CORIDOR (Collection of Online Resources & Inventory Database) is a CDC sponsored online resource portal designed to advance chronic disease prevention and health promotion activities by providing easy access to credible, evidence-based and practical chronic disease prevention tools in one location.

 

Microsite®Million Hearts

This microsite includes Million Hearts® evidence-based protocols, action guides, and other quality improvement tools to help clinicians and other health professionals reduce their patients’ risk for a heart attack or stroke. Find a place to embed the microsite on your website to stay up-to-date on Million Hearts® resources.
 

Diabetes at Work website

The user-friendly design features a responsive rotating slider on the homepage, a new drop-down toolbar for each selected topic, and an updated Quick Links section. Your favorite resources like Lesson Plans and Diabetes Basics are still there to help your company prevent and manage diabetes in the workplace.

American Diabetes Association provider videos

The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length.

Resource toolkit

As part of the patient’s health care team, pharmacists can improve disease outcomes by improving medication adherence and providing patient planning, education and counseling. The Community Preventive Services Task Force found strong evidence that team-based care can improve blood pressure control when a pharmacist is included on the team. A new toolkit from The University of Colorado provides sample agreements and resources helpful in integrating clinical pharmacists into the care team. Click for more information.

Updates in Pharmacist Cardiovascular Care online study modules

The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, with support from the Colorado Department of Public Health and Environment, created a series of online educational modules that highlight the role of pharmacists in the prevention and treatment of cardiovascular disease. To view the modules:
  1. Navigate to ipharmCE.UCDenver.edu (or http://ipharmce.ucdenver.edu/student/exam). You will need to create a profile if you do not have one. To create a profile, enter your email under “New Account Sign-up,” and follow the instructions on the screen.
  2. Once logged in, click “View All Programs” under Home Study Activities on the left side of your screen.
  3. Under the group Updates in Pharmacist Cardiovascular Care, click on the individual presentation titles.
