Our Suspicion of Sepsis insights dashboard featured in the Long Term Plan 

Our Suspicion of Sepsis Insights dashboard has been featured by NHS England as part of the NHS Long Term Plan to reduce the toll of ‘hidden killer’ sepsis. The dashboard for the first time enables organisations to see an overall picture of hospital admissions coded in the SOS category, allowing them to assess the scale at a local, regional, and national level. The dashboard has currently been adopted by five other AHSNs, with others coming on board and there is also added international interest; demonstrating the power of our networks for the spread of innovation to benefit patients. Read more... 
Our evaluation revealed that, while undiagnosed, rare disease patients have cost the NHS in excess of £3.4 billion

We have published our research, commissioned by healthtech company Mendelian, which for the first time, shed light on the cost and resource impact the lengthy process of diagnosing rare diseases is having on the NHS. Our Head of Data and Analytics, Julia Wilkins, has shared her reflections about her research findings and the opportunities it has given to the NHS as she presented to the All Party Parliamentary Group on rare, genetic and undiagnosed conditions. One year on: Discover goes from strength to strength!
Just over a year ago we launched Discover, a new register of people living in North West London interested in getting involved with health research opportunities relevant to them. By joining the register, patients and the public are able to get involved with research studies that could contribute to a healthier future for everyone. Discover, which is also linked to the WSIC database, also offers a range of services to researchers including retrospective and real world studies, as well as the opportunity to assess whether their studies are feasible to carry out in North West London. Leading the sector with our population health learning labs 

We are proudly leading North West London in bringing together a diverse range of people working across health and social care through our population health learning labs. The labs are focused on covering key themes and topics on person-centred coordinated care, crossing healthcare, local government and the voluntary sector. 

The first lab was held this month on 'Leadership within Integrated Care Systems' attended by over 80 people to hear from an expert panel and chair to build, join and drive the conversation in this space. 

The next learning lab is on Thursday 16 May on citizen engagement and co-design.

Proudly supporting building trust in data sharing

The conversation is growing in London around approaches to take that will support efforts to build trust in data sharing. We held a seminar for policy officials, key opinion leaders and representatives from the One London Local Health and Care Record Exemplar (LHCRE) team to explore how deliberative engagement approaches could support building trust in data sharing.
 
We heard from Dr Natalie Banner of Understanding Patient Data, Dr Michael Burgess from University of British Columbia and Dr Mary Tully and Malcolm Oswald from the Connected Health Cities. 
 
There were fruitful discussions both about the benefits and limitations of such an approach, with many concluding that even with limitations it is one of the best methods available to tackle complex issues with numerous trade offs such as data sharing. We are using the insight gained from this event to inform future work of the LHCRE team.

Medicaid Learning 

We were fortunate enough to visit one of the Medicaid Performing Provider System (PPS) on Staten Island, New York recently. As a local coordinator of value based care for the local Medicaid population of 70k, the small central PPS team has made remarkable progress within a couple of years in areas such as reducing hospital admissions and visits. For example, using data to pinpoint frequent visitors to paediatric A&E for asthma related conditions and working with the local community, families and clinicians inpatient hospitalisation more than halved. An important element of the intervention was the focus on socio economic factors such as clean and quality housing. These interventions were possible through initial capital given to the PPS that could be deployed flexibly. The sharing of savings helped to align all providers in the health eco-system. As in many other successful transformation programmes, data and focus on a small number of high priority areas, upfront investment and shared incentives formed the crucial trio.  Contact us for more information. 
Investors’ work with EIT Health

As part of ICHP’s work with EIT Health Investors’ network we held an event bringing together investors in London to be recruited to the network. Events like this are vital to engage with this important community in healthcare and for us to inform discussions ranging from the risks of cross border investment to the challenges of scaling in a market with so many healthcare systems.

Significant milestone in Harrow

After supporting a large piece of co-design work at the end of last year, we have begun prototyping a new model of care in a Harrow care home. The programme aims to implement a shared care record within the home, a single assigned GP for all residents, use of a single point of access for acute/primary care needs and enhanced training for staff. It will also use a residents forum to understand how we can improve well-being and happiness as well as a staff forum across Harrow to better share learning (and signpost to underused existing resources).
 
The programme will be closely monitoring impact on a number of metrics including hospital admissions, call-outs and staff/ resident experience. The home we are supporting has some of the highest LAS activity per head in London. 
 
Learnings from the prototype will go on to inform a model that is intended to be scaled across Harrow as part of their wider integrated care programme.

National Programmes
NHS Trust first in the country to translate ‘ESCAPE-pain’ chronic joint pain programme into Gujarati

ESCAPE-Pain is one of the seven AHSN programmes developed regionally which have been selected for national adoption and spread across the AHSN Network. We have supported Brent CCG and London North West University in rolling out the programme and to be the first in the country to translate ESCAPE-pain into Gujarati. We are proud to demonstrate, how through our AHSN network, we can adapt these programmes to meet and respond to our local population.

PINCER implementation leader

We are one of the first AHSNs to start implementing PINCER this year! Currently seven out of eight CCGs are undergoing training in using PINCER to benefit patients. The first downloads of medication errors identified are now expected, with a rolling schedule based on GP and pharmacist capacity until the end of March 2020.

This AHSN national programme PINCER aims at reducing the risk of medication errors occurring. It is an audit tool that searches the GP Practice computer system and identifies patients who are being prescribed medicines that are commonly and consistently associated with medication errors.  There is then a pharmacist intervention i.e. reviews and makes recommendations to the GP, on the most appropriate action.

Transfer of Care Around Medicines (TCAM)

We are currently supporting three NWL Trusts in implementing this AHSN national programme which aims to ensure continuity of care for patients discharged from hospital back home. When patients discharged from hospital are identified as needing extra support, they are referred through a safe and secure digital platform for advice from their local community pharmacist. The three Trusts are due to start sending the patient discharge summary to community pharmacists in June with further Trusts due to begin implementation in Autumn.

PReCePT upping the ambition 

In North West London as a region we have surpassed the national PReCePT programme's ambition to achieve uptake of magnesium sulphate (MgSO4) in 85% of eligible mothers, we now have our eyes set on the stretch ambition of 95%. Four of our six maternity units have consistently achieved 100% uptake over the past two months and will be receiving certificates from our Regional Clinical Lead. 

PReCePT is a national programme designed to help reduce cerebral palsy in babies through the increased antenatal administration of magnesium sulphate MgSO4 to mothers during preterm labour.

Helping to enhance the mental health landscape across North West London

We have making great bounds with Serenity Integrated Mentoring (SIM) which is due to go live across three sites (Westminster, Hillingdon and Hounslow) at the end of the month. This has included us supporting the training of over 70 people from mental health Trusts and the police force in SIM. 

More wider we have also been commissioned by the Greater London Authority to design an intervention to increase mental health access for rough sleepers in London and are reviewing the Crisis care pathway for West London NHS Trust.

Working wider
Outpatient referral dashboard

We have been working with our CCG partners in NW London to design a dashboard for GP first outpatient referrals. The dashboard has now gone live for four CCGs  with the next four CCGs adopting soon.

This dashboard is designed as an engaging learning tool which supports users to explore the data around GP-led first outpatient referrals and enables practices across North West London to identify and drive areas for improvement. This has involved working closely with Clinical Leads and other CCG colleagues to understand the requirements for the dashboard and visualisation, then working with our Business Intelligence to turn this into reality, with a real focus on a user-friendly, engaging approach.

Brent e-hub live! 

The Brent Online Consultations e-hub went live this month; which is part of the GP forward view online consultations programme and is the first at-scale delivery model for online consultations to be set up by the NHS. So far, approximately 580 patients are visiting the online platform, powered by eConsult, every week. ICHP have been supporting this programme from the start and will be running the evaluation in the new year to understand the impact this has had on patients and GP Practice staff. Brent CCG has been recognised for their innovative thinking by Healthy London Partnership and North West London CCGs and have been selected to be the Digital Accelerator site for our STP, bringing more funding to develop this model further. ICHP will continue to support this programme into the new year.

Cardiology up-skilling events saving lives
Our RCGP accredited series of Cardiology training sessions have seen us up-skill nearly 150 GPs, nurses, pharmacists as we support health care professionals to manage their patients and their conditions. Joined by Dr Yassir Javaid and Dr Ravi Assmoul (local cardiologist to NWL) who have been sharing their expertise and the local opportunities to improve care.  It was brilliant to hear that already a local clinician has detected AF in a patient since using the skills they learnt from attending these sessions. Learning, sharing and connecting for our members
Our Business Intelligence team have been part of the growing health discussions with leaders from Switzerland, the Netherlands, representatives from the National Health Fund and World Bank in Poland. This has included the importance of Patient Reported Outcome Measures (PROMs) in helping shape service quality, engaging patients and evaluations. Also on Integrated Care Systems, population health, business models and impactibilty. Inspiring the next generation 
Our Innovation Delivery Lead, Matthew Chisambi, ran a workshop with Healthwatch Central West London at Hammersmith & Fulham’s Youth Council. The aim was two-fold, explore young people’s thoughts on digital health and also to design the schools-based project Matthew is hoping to set up. The session was a success and the Youth Council selected Sexual Health as their first priority area and now have 4/5 projects designed by young people that we will be consolidated. Contact Matthew for more information. 

We have also been supporting ten sixth form students to actively volunteer in two care homes across Brent with 13 residents as part of our Brent Loneliness and Technology project. Read more...

Supporting the Medtech super connector!
We proudly supported the Medtech hackathon with our partners helping to shape an innovation call on mental health. We set challenges and supported mentor teams for innovators. The weekend was incredibly successful, and provided us with an opportunity to support both our partner and also local innovators to address un-met needs and be part of shaping solutions to local challenges.Contact Omar Butt, Innovation Account Lead for more information.

Over the past year we have been working with maternal neonatal teams to consistently work on their QI projects to develop and test out change ideas. Wider network updates
Applications for a London-based digital health Accelerator are open, as part of efforts to increase NHS and patient access to cutting-edge technologies.

The DigitalHealth.London Accelerator is targeting innovative healthtech businesses, and inviting them to apply to receive help to develop and deploy digital solutions to some of the NHS’ most pressing challenges.

This is the fourth year the initiative has operated in London, where it began in 2015 in response to a challenge by the Mayor of London to make London a global hub for health and technology. The call comes on the back of the recent publication of the NHS Long Term Plan, which focuses on ways to ensure the NHS is “fit for the future,” including a push around digital technology. This priority is also echoed in the Topol Review, which was published earlier this month.

World-best innovations supporting Mental Health and early diagnosis and prevention of cancer join NHS Innovation Accelerator

The thirteen innovations joining the NIA have been announced as part of the fourth year of this award-winning national accelerator. We are proudly part of this NHS England initiative delivered in partnership with our AHSN networks, the NIA has supported the uptake and spread of 37 high impact, evidence-based innovations across more than 1,700 NHS sites since it launched in July 2015.

The AHSN Network strongly supports the findings of the newly published Topol Review, which sets out how to prepare the healthcare workforce to deliver the digital future.

The independent report, on behalf of the Secretary of State for Health and Social Care, states that 90% of NHS jobs will require digital skills within 20 years. In looking at how technological and other developments are likely to change the roles and functions of clinical staff the review has delivered three key principles:

Through its established links with citizens, industry, health and social care and academia, the AHSN Network is well placed to support staff to adopt these new solutions, building on its reputation as the engine for innovation in the NHS.

"Role of AHSNs to simplify pipeline for healthcare innovation highlighted in NHS Long Term Plan"

With its clear emphasis on research and innovation for patient benefit, the NHS Long Term Plan  is warmly welcomed by the AHSN Network.

Heralded as the blueprint to make the NHS fit for the future, the Long Term Plan highlights investment in latest technology, digital health and cutting edge treatments, coupled with early detection and a renewed focus on prevention to stop an estimated 85,000 premature deaths every year.

It also lays out plans for a more integrated, person-centred service, where health bodies come together across primary, community and mental health to provide better, joined up care in partnership with local government.

AHSNs will also link ever more closely with other regional support, such as Rightcare and Getting It Right First Time (GIRFT), to ensure adoption of innovation and service improvement are addressed in tandem.

Digital health Standards

NHS England, NICE, Public Health England, MedCity and DigitalHealth.London have worked together to develop a framework to support digital health innovators, grant funders and investors, and commissioners to understand what a ‘good’ level of evidence looks like.

The NICE Evidence Standards Framework was launched on Monday 10 December and aims to help people provide evidence of effectiveness and economic impact.

A new code of conduct for artificial intelligence and other data-driven technologies published

The code encourages technology companies to meet a gold-standard set of principles to protect patient data to the highest standards. It has been drawn up with the help of industry, academics and patient groups.

The aim is to make it easier for suppliers to develop technologies that tackle some of the biggest issues in healthcare, such as dementia, obesity and cancer. It will also help health and care providers choose safe, effective and secure technology to improve the services they provide.
