Million Hearts® Hypertension Control Challenge
The Million Hearts® Hypertension Control Challenge, a key part of the Million Hearts® 2022 initiative, identifies and highlights clinicians and teams helping their patients achieve blood pressure control. Since 2012, Million Hearts® has recognized 101 Hypertension Control Champions in 35 states for their success in getting at least 80 percent of their hypertensive patients’ blood pressure under control. These experts have shared the keys to their success: a combination of priority-setting, team delivery of treatment protocols, staff incentives, out-of-office blood pressure monitoring, and more. You and your teams are now challenged to join this stellar group of high performers by submitting your application for the 2019 Million Hearts® Hypertension Control Challenge, open to solo and group practices and health systems that achieve hypertension control rates of at least 80 percent for a 12-month period. Applications can be submitted until April 1, 2019.

Screening for social determinants of health in populations with complex needs: implementation considerations
With the recognition that social determinants of health can account for up to 40 percent of individual health outcomes, particularly among low-income populations, their providers are increasingly focused on strategies to address patients’ unmet social needs (e.g., food insecurity, housing, transportation, etc.). This brief from the Center for Health Care Strategies examines how organizations participating in Transforming Complex Care, a multi-site national initiative funded by the Robert Wood Johnson Foundation, are assessing and addressing social determinants of health for populations with complex needs.

Doctors and racial bias: still a long way to go
Read this article in the New York Times highlighting a systematic review published by the Academic Emergency Medicine that gathered all the research on physicians that measured implicit bias with the Implicit Association Test and included some assessment of clinical decision making.

Depressive symptom dimensions and medication non-adherence in suboptimally controlled type 2 diabetes
Research suggests differential effects for somatic and cognitive-affective depressive symptoms in predicting health outcomes. Read the study published by the Journal of Diabetes and its Complications, which evaluated differential relations with medication non-adherence among disadvantaged, and predominantly immigrant adults with sub-optimally controlled type 2 diabetes.

Opportunities for complex care programs to address the social determinants of health
In recognition of the underlying social, economic, and environmental factors that contribute to poor health outcomes, many complex care programs are incorporating strategies to address the social determinants of health. This brief from the Center for Health Care Strategies explores opportunities to better meet patients' social needs, including identifying patients' non-medical needs, employing non-traditional workers, partnering with community-based organizations and social service agencies, testing new uses for technology to help address social needs, and identifying sustainable funding to support non-medical services.

The Community Clinical Linkages Toolkit

The Million Hearts® Collaboration, American Heart Association and other stakeholders have unveiled a new tool to promote community-clinical linkages: The Community Clinical-Linkages Toolkit. The Toolkit organizes an array of resources in a handy grid format to help public health practitioners implement the seven step-by-step strategies that are highlighted in the Community-Clinical Linkages Practitioner’s Guide under the LINKAGE framework.

American Diabetes Association provider videos

The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length.

Resource toolkit

As part of the patient’s health care team, pharmacists can improve disease outcomes by improving medication adherence and providing patient planning, education and counseling. The Community Preventive Services Task Force found strong evidence that team-based care can improve blood pressure control when a pharmacist is included on the team. A new toolkit from The University of Colorado provides sample agreements and resources helpful in integrating clinical pharmacists into the care team. Click for more information.

Updates in Pharmacist Cardiovascular Care online study modules

The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, with support from the Colorado Department of Public Health and Environment, created a series of online educational modules that highlight the role of pharmacists in the prevention and treatment of cardiovascular disease. To view the modules:
  1. Navigate to ipharmCE.UCDenver.edu (or http://ipharmce.ucdenver.edu/student/exam). You will need to create a profile if you do not have one. To create a profile, enter your email under “New Account Sign-up,” and follow the instructions on the screen.
  2. Once logged in, click “View All Programs” under Home Study Activities on the left side of your screen.
  3. Under the group Updates in Pharmacist Cardiovascular Care, click on the individual presentation titles.
