Coordinating care to older people with complex needs in the home environment
Seminar: Coordinating care to older people with complex needs in the home environment with Dr Nick Goodwin
Join us on Wednesday, 17 April 2019 at University of Newcastle Campus in Sydney for a free to attend seminar with Dr Nick Goodwin, who will consider what works and how can we improve care delivery for older people with complex needs at home.

Older people with complex health and care needs are often beset by challenges in accessing appropriate care and encounter problems of poor care co-ordination on a daily basis. Using international case examples, this seminar will examine a continuum of approaches that have been used to help improve quality of care and quality of life to older people living at home, including to their carers and family members. These approaches include ways that enable older people to live well and independently in the home environment; promote access to primary and community-based resources in the community; enable effective and seamless care transitions; and support dignity in care for people with dementia and nearing the end of life. The seminar concludes with a review of the newly-published roadmap from the EU project SUSTAIN that was first presented at their final conference in Brussels on the 13th March 2019. The roadmap sets out guidance on how to design, implement and improve integrated care to older people including hints and tips on creating an enabling environment for change.

Seminar to be followed by the launch of the 2nd Asia-Pacific Integrated Care Conference to be held in Melbourne in November and a short discussion around IFIC Australia activities.

Registrations now open for ISSIC19

The International Foundation for Integrated Care (IFIC) is delighted to announce registration is now open for its popular Summer School which returns next June. ISSIC19 will take place in the heart of Oxford, at Wolfson College, part of the University of Oxford. Summer School begins on Sunday, 23 June, 2019, with a welcome dinner and will run until Friday, 28 June.

This course has been developed for those tasked with designing, implementing, leading and/or managing integrated care and want to learn more about the evidence-based tools and instruments available to them from around the world.

It is aimed at professionals in the field of integrated care, clinicians, researchers and managers, and offers an intensive one-week, stand-alone education and training course on the theory and practice of Integrated Care. It can also contribute towards CPD.

During the course participants will hear from a series of experts in the Integrated care field, including international guest speakers. The syllabus will include a site visit, presentations, case studies and discussions around all aspects of implementing integrated care in your organisation and also offers the unique opportunity to foster international networking and knowledge exchange.

In feedback from this year’s Summer School every respondent said they would recommend the course.

Professor Yves Couturier, University of Sherbrooke said:
“Even though I have been involved in research in Integrated care for the past 15 years, I learned a lot at the Summer School. It gave me the chance to discuss international cases studies that were highly relevant to my own work and I have returned with renewed energy.”
