CDC Guideline for Prescribing Opioids for Chronic Pain
After protracted debate over federal guidelines for prescribing opioids, the Centers for Disease Control and Prevention has clarified its position, stating that its three-year-old recommendations were not intended to deny clinically appropriate treatment to patients who suffer acute or chronic pain from conditions such as cancer and sickle cell disease. Those guidelines, which focus on chronic pain except for cancer and end-of-life care, target primary care physicians because family doctors write the vast majority of prescriptions for painkillers.

CoPay Update
There has been an increasing number of Medicaid beneficiaries who are being turned away for inability to pay a copay, especially when trying to fill prescriptions. Some pharmacies do not use the KyHealthNet/MMIS system, so they aren't seeing the copay or poverty indicators.
DMS will be sending out a letter to pharmacists soon to remind them that no one at/below 100% FPL should be turned away and that even those above 100% FPL should get a 72-hour supply of medicine, if it is medically necessary.

Kentucky's Medicaid expansion boosts colon cancer screenings, study says
That’s a significant increase for a state that has the highest rate of colon cancer incidence in the country, according to the Centers for Disease Control and Prevention. While more Kentucky Medicaid patients were diagnosed with colon cancer after the expansion - a 132.4% increase - researchers found more of them survived. This was especially true in eastern Kentucky, where Medicaid coverage of Appalachian patients increased by 199%.

Kentucky students aren’t getting all the shots they should have. What the stats show.
The latest statewide immunization survey of Kentucky students showed that the percentage of kindergartners who received required immunizations against diseases ranging from chickenpox to hepatitis B was below the state’s goal of 95 percent.
University of Kentucky pediatric infectious diseases associate professor Sean McTigue is concerned about that data, including that the 2018 state immunization survey showed that only 91.6 percent of kindergartners were immunized against chicken pox.

Uniform Data System Updates
- When Will the 2018 Uniform Data System (UDS) Data Be Ready?
HRSA has received several inquiries about the timing of the 2018 UDS data release. We are reviewing the data included in the health center reports and expect to release the data in August on HRSA’s website.
- UDS Reporting Survey
As part of the UDS Modernization Initiative, new functionalities within the EHBs and new offline reporting features were introduced for the 2018 reporting period. We invite all health centers to provide feedback for improvement through a short survey, which should take no more than five minutes to complete. Participation is voluntary and all responses are anonymous. The survey will close on Friday, May 3.
- 2019 UDS Reporting Changes TA
HRSA presents a TA webinar that will provide an overview of the changes for the calendar year (CY) 2019 UDS data collection and reporting activities for reports submitted by February 15, 2020. We will also discuss possible changes for 2020 UDS and revised clinical measures resulting from continued alignment with electronic specified clinical quality measures. New Supplemental Funding Opportunity Available: FY 2019 Integrated Behavioral Health Services
On Friday, March 29, HRSA announced the availability of $200 million in supplemental funding to health centers for Integrated Behavioral Health Services (IBHS). IBHS funding will increase access to high-quality integrated behavioral health services, including prevention or treatment of mental health conditions and/or SUDs, including opioid use disorder (OUD).
Eligible health centers will receive emails announcing the availability of the Electronic Handbooks (EHBs) application module on Friday, April 5. Applications are due in EHBs by Monday, May 13, at 5:00 p.m. ET.

New Funding Opportunity: Oral Health Infrastructure
On Friday, March 22, HRSA announced the availability of $76 million in one-time supplemental funding for Oral Health Infrastructure (OHI) to support health center infrastructure enhancements. These investments will increase access to new or enhanced high-quality, integrated oral health services, including those provided via telehealth and virtual dentistry

National Health Service Corps develops new Rural Community Loan Repayment Program
National Health Service Corps (NHSC) officials have announced that a new financial assistance program for health care providers -- the Rural Community Loan Repayment Program -- will accept applications later this spring. The program is designed for professionals working to combat the opioid epidemic in the country's rural communities.
Developed in coordination with the Federal Office of Rural Health Policy's Rural Communities Opioid Response Program initiative, the NHSC's new program seeks to expand access to evidence-based substance use disorder treatment, assist in recovery, and prevent overdose deaths.

National Health Service Corps Scholarship Program
Through the Health Resources and Services Administration, the National Health Service Corps (NHSC) will provide tuition, fees, and monthly stipend for full-time enrollment in primary health care professions training for up to four school years. In exchange, NHSC scholars will commit to serving in a high-need urban, rural, or frontier community upon graduation.

Pharmacy Update: Communicate Effectively
Effective communication is born out of effective listening. Start with open-ended questions which should allow one to learn more about a patient’s specific difficulties and tailor any counseling to that specific situation. It is important to check your patients’ understanding of medication instructions. Research has shown that patients are three times more likely to not take their medications when their providers did not have a collaborative communication style. Patients were also six times less likely to take their medications when a healthcare provider didn’t ask about social issues such as housing, employment and partner relationships.
Misconceptions, poor communication keep eligible adults from statin use-According to a new report in the Journal of American Heart Association many patients who could benefit from taking statins have never been prescribed them, and those who were offered or initiated treatment don’t take them now because of side effects or fear of side effects. Patient surveys were used to determine why they underutilized statins including non-initiation, refusal, and discontinuation. Of those not on a statin, 59.2% reported never being offered a prescription; while 10.1% declined a statin; and 30.7% had discontinued therapy. The most common reasons listed for declining or discontinuation statins were fear of side effects and perceived side effects. The study also reported responses that many people perceived statins as unsafe (70.4% of current users vs. 36.9% of those who declined and 37.4% of those who discontinued) or ineffective (86.3%, 67.4% and 69.1%, respectively). The willingness to take a statin was high at 67.7% of those never offered, and 59.7% of patients who discontinued a statin said they would consider initiating or retrying a statin. The study did accept the possibly that some people may not have remembered being offered a statin so that may have been a reason for overestimation of percentage of people who were never offered one. However, if a patient does not remember the conversation, maybe the discussion was not an effective one.

Pharmacy Access Office Hours are a monthly opportunity for health center and PCA staff to ask questions and share information about pharmacy-related issues access activities. They are supported by NACHC’s cooperative agreement with BPHC, and are held the third Thursday of each month at 2:00pm Eastern. This month’s session will include a brief presentation about reimbursement, 340B, and related issues involving clinic-administered drugs. Ceiling price for drugs purchased under 340B is now available on HRSA database
Effective Monday April 1, health centers and other 340B-eligible providers can verify the “ceiling price” for all the drugs they purchase under 340B. The ceiling price is the maximum amount that a manufacturer may charge for a drug; however, it may not match a health center’s actual cost for a drug, as it does not include fees paid to distributors, or additional discounts that the manufacturer offers voluntarily.
