Leading the national conversation on Sepsis

‘The value added by ICHP in terms of learning and embedding approaches which improve patient safety is hard to quantify"

We had a fantastic turnout at our National Sepsis Data Summit in April, where we brought together a wide range clinical experts, policy leaders, coders (from NHS England, professional associations, NHS Digital and the Care Quality Commission) to discuss the next steps in the development of the Suspicion of Sepsis dashboard and our drive of improving the quality of sepsis data locally, regionally and nationally.

Developed through cross system collaboration the Suspicion of Sepsis dashboard enables NHS staff to use reliable data, to monitor and assess the long-term impacts of quality improvement initiatives on patients who are admitted into hospital due to bacterial infection or Sepsis over time. 

We are also proud to announce that our SOS dashboard has been shortlisted for a Health Service Journal Patient Safety Award 2019 in ‘Best Emerging Solution for Patient Safety.

Celebrating a year of reducing cerebral palsy in pre-term babies in NW London 

We are proudly leading the way in supporting unprecedented numbers of mothers to take preventative measures against cerebral palsy in NW London through our PReCePT team.

For the past year ICHP has been working with all six maternity and neonatal units in NW London to deliver the national PReCePT programme, which aims for a 95% take-up rate of MgSO4 by 2020. 

Already through the programme in NW London there is a 96% uptake rate of MgSO4 amongst eligible mothers during preterm labour, which is significantly reducing the risk of cerebral palsy for pre-term babies in the region. 

PReCePT is a national programme designed to help reduce cerebral palsy in babies through the increased antenatal administration of magnesium sulphate MgSO4 to mothers during preterm labour.

Discover register growth accelerating 

ICHP is pleased to announce that through our Discover research register programme we now become a member of the Association of Medical Research Charities (AMRC). This membership will help us build closer relationships with medical research charities and their beneficiaries as we look to build future research collaborations and grow the range of studies on our portfolio, and also continue to develop research that addresses the needs of patients.

Our Discover research register allows us to connect researchers with consented patients who have expressed an interest in participating in medical research, from one of Europe’s largest people-and-patient-centred longitudinal data sets. In just one year since launch nearly 4,000 people have joined, with a growing pipeline interest in developing research studies.

The Discover Research Access Group (DRAG) has now also approved 17 research studies. This includes an academic study led by Dr Sofia Pappa at West London Hospital Trust to investigate asthma/COPD and rates of depression, anxiety and smoking and a study to investigate the impact of a new community heart failure service in Ealing led by the Cardiology team at Imperial College Hospital NHS Trust.

NEWS rolled out in patient assessments

We have been working with community teams in London North West University Healthcare NHS Trust (LNWUHT) and Central London Community Healthcare NHS Trust to introduce the National Early Warning Score (NEWS) in their patient assessments.

This scoring system helps clinicians identify clinical deterioration and provide a universal language to communicate acuity across different settings. LNWUHT have now completed a one-month pilot in the Short-Term, Assessment, Rehabilitation and Reablement team (STARRS), and achieved NEWS assessments in 93% of their patients (from a baseline of 0%).

Reducing waiting times in north west London

ICHP has been supporting pathway design at London North West University Healthcare NHS Trust.

Our Head of Provider Portfolio, Craig, has been heading up the team, working alongside clinicians and managers in the redevelopment of both the two-week wait (2WW) pathway for gynaecology and the fractured neck of femur pathway, across all three sites.

The work is aimed at shortening waiting times for the 2WW pathway and improving length of stay for the neck of femur work.

Upskilling health professionals with accredited training 

ICHP have specifically designed for NW London a series of free RCGP accredited training programme for primary care health professionals (GPs, nurses and pharmacists). This training has seen over 100 attendees upskilled to enable better diagnosis and management of patients with cardiovascular conditions in the community. 

They were joined by Dr Yassir Javaid and Dr Ravi Assmoul (local cardiologist to NWL) who showed GPs their local data and their opportunity around Stable coronary disease, Angina, CV risk  and interpreting ECGs.

SIM programme live in north west London

We are proud to announce SIM (Serenity Integrated Mentoring) has gone live in NW London.

The programme trains a police officer with a passion about tackling mental health, and parachutes that officer into their local community mental health team to assist with the clinical and risk management of people with the most challenging problems.

ICHP introduced and implemented SIM in partnership with West London NHS Trust and Central and North West London NHS Foundation Trust and we now have three police officers working with the programme in Hillingdon, Westminster and Hounslow.

Improving transfer of medicines from secondary to primary care

In May we held two events to launch our Transfer of Care Around Medicines (TCAM) programme in NW London, which aims to better ensure that patients leaving secondary care can receive additional support with their medicines from community pharmacies if needed.

We have partnered with The Hillingdon Hospitals NHS Foundation Trust, Royal Brompton and Harefield NHS Foundation Trust and Chelsea and Westminster Hospital NHS Foundation Trust for the launches, with 166 representatives from both hospital and community pharmacists attending both events.

It was great to have David Tamby Rajah, Regional Lead for Dental, Optometry and Pharmacy, from NHS England, at the second launch event supporting the programme.

Leading conversation on data in healthcare

Representatives from across the healthcare system joined us at our Intersections event, that we held in partnership with Draper Esprit, to discuss data, power, and the role of the public in our health system.

This event brought together healthcare providers, patients, technology specialists, and startups for a conversation about data, AI and, most importantly, the role of the public.

Events concluded with a fireside chat between Axel and Lord Paul Drayson, as they discussed the ethics of responsibly using patient data.

These discussions are building on the growing conversation in London around the best approaches to support efforts to build trust in data sharing. ICHP is using this insight gained to inform our work with the One London Local Health and Care Record Exemplar (LHCRE) team.

ICHP shares knowledge on understanding the implications of data

Members and clients of ICHP were invited to a training day with our business intelligence team to freely learn more about analytics software, Tableau.  This software allows users to interactive data and create visualization so rather than presenting data in lists or static charts, charts created in Tableau present information in a way that is easy to understand and therefore to act on. 

The British Heart Foundation and North East Quality Observatory Service (NEQOS) were amongst those who we helped get to understanding the possibilities of these tools for their work and help better visualise their data.

Association of British HealthTech Industries

Axel Heitmueller, Managing Director, was invited to join an Association of British HealthTech Industries (ABHI) delegation to Texas to promote British companies and also prepare a visit by Lord Prior, current chair of NHS England and Chairman of University College Hospital, later this year.

ICHP has close relationships with Dell Medical School in Austin and the delegation met with Prof Elizabeth Teisberg, who is part of the ICHP advisory group. Prof Teisberg is working on how to make value-based care a reality in health systems, an aim for ICHP in NW London.

Examining AI in the future of health care

Our Innovation Delivery Lead, Abeer Itrakjy, took up an invitation to travel to Boston for the World Medical Innovation Forum 2019. Abeer joined fellow experts in conversation on cutting edge health innovation including big data and AI. 

Following the conference, we are working with our radiology colleagues in NW London to look at how we can pilot the use of AI technology here with our members.

Improving local access to care with estate planning  

We are working with the NHS Property Board, NHS England and NHS Improvement, to help people across London receive better care closer, or in their own homes, starting with a test site in North Central London.

The project aims to develop a Primary and Community Care Estates Delivery Toolkit, to support the local implementation of estate strategies and create better cases for funding to support estate development. 
 
ICHP is supporting as part of two phases, firstly to assist in scoping the intended content of the toolkit, to ensure it is aligned with local estate planning needs and the complex range of national partners involved. Once complete, ICHP will assist in the development of the toolkit by embedding a data-driven, population health management approach in an exemplar site in North Central London.
