$5 Million in New Grants Accelerate Transformation in Care
The John A. Hartford Foundation (JAHF) is funding ground-breaking efforts to improve care for older adults through four grants totaling nearly $5 million:
The National Academy for State Health Policy will develop a Resource and Dissemination Center to support the work of the congressionally authorized RAISE Family Caregiver Act Advisory Council.
The Center to Advance Palliative Care will take on four interrelated projects to ensure more seriously ill older adults have access to quality palliative care across settings.
