New guidelines released for primary prevention of cardiovascular disease
The American Heart Association and the American College of Cardiology released new guidelines emphasizing the need for team-based care and a healthy diet, including reduced dietary cholesterol and sodium, to achieve good health and prevent cardiovascular disease. Annual calculations of body mass index and waist circumference, brisk daily activity and avoidance of tobacco products also is important to prevent CVD, according to the guidelines. Guidelines additionally call for evaluation of social determinants of health that affect individuals to inform treatment decisions. Read the updated guidelines and get highlights here.

Community Preventive Services Task Force (CPSTF) recommends pharmacy-based interventions for cardiovascular disease prevention
Patients taking their blood pressure medication as prescribed is associated with 45 percent greater odds of controlling their blood pressure compared with not taking the medications properly. Improvements in blood pressure control may decrease the burden of heart disease and stroke. 

Additionally, patients with high cholesterol who do not adhere to their medications have a 26 percent greater likelihood of a cardiovascular-related hospitalization compared to patients who adhere to their prescriptions.

CPSTF recommends tailored pharmacy-based adherence interventions for cardiovascular disease (CVD) prevention. Evidence shows interventions delivered or guided by pharmacists increase the number of patients who report taking medications as prescribed.

Governor welcomes Diabetes State Engagement Meeting attendees
Governor Jared Polis took time to record a welcoming message for attendees of the Diabetes State Engagement Meeting on August 20. One-hundred-ten people attended the event, which focused on opportunities to promote the health and success of people with or at risk of developing type 2 diabetes. “As experts in this field, I turn to you to help engage in this conversation and develop strong solutions—be bold!—that will help Colorado lead,” said Polis. On August 21, a subset of attendees met to determine action steps that will form the basis for a two-year plan to increase access to, availability of and coverage for the National Diabetes Prevention Program and Diabetes Self-Management Education and Support programs.

Rural telehealth toolkit
The Rural Telehealth Toolkit is designed to help rural communities identify and implement telehealth programs. Developed by the National Opinion Research Center (NORC) on behalf of the Federal Office of Rural Health Policy (FORHP), the toolkit compiles promising approaches and resources to support rural communities seeking to develop and expand telehealth programs. This toolkit presents practical strategies that can help rural communities identify successful telehealth models, address common implementation challenges and prepare for long-term sustainability.

The Community Clinical Linkages Toolkit

The Million Hearts® Collaboration, American Heart Association and other stakeholders have unveiled a new tool to promote community-clinical linkages: The Community Clinical-Linkages Toolkit. The Toolkit organizes an array of resources in a handy grid format to help public health practitioners implement the seven step-by-step strategies that are highlighted in the Community-Clinical Linkages Practitioner’s Guide under the LINKAGE framework.

American Diabetes Association provider videos

The American Diabetes Association, in partnership with the Colorado Department of Public Health and Environment, Kaiser Permanente, Telligen, and the Colorado Prevention Alliance developed a series of short webinars for health care professionals. There are three sections: diabetes prevention, diabetes self-management education, and diabetes and depression. The videos are available online at no cost and run between 5 and 15 minutes in length.

Clinical pharmacy resource toolkit

As part of the patient’s health care team, pharmacists can improve disease outcomes by improving medication adherence and providing patient planning, education and counseling. The Community Preventive Services Task Force found strong evidence that team-based care can improve blood pressure control when a pharmacist is included on the team. A new toolkit from The University of Colorado provides sample agreements and resources helpful in integrating clinical pharmacists into the care team. Click for more information.

Updates in Pharmacist Cardiovascular Care online study modules

The University of Colorado Skaggs School of Pharmacy and Pharmaceutical Sciences, with support from the Colorado Department of Public Health and Environment, created a series of online educational modules that highlight the role of pharmacists in the prevention and treatment of cardiovascular disease. To view the modules:
  1. Navigate to ipharmCE.UCDenver.edu (or http://ipharmce.ucdenver.edu/student/exam). You will need to create a profile if you do not have one. To create a profile, enter your email under “New Account Sign-up,” and follow the instructions on the screen.
  2. Once logged in, click “View All Programs” under Home Study Activities on the left side of your screen.
  3. Under the group Updates in Pharmacist Cardiovascular Care, click on the individual presentation titles.
