Community Connector Service
Supporting the Health and Disability Community Impacted by COVID-19
The Needs Assessment and Service Coordination Service is for people with a physical, intellectual and or sensory impairment, or a disability that is likely to last longer than 6 months and reduces their ability to function independently.
As part of the Ministry of Health's Disability Support Service initiative to support NASC, a National Referrals Centre is being piloted and is now working alongside NASC Hawkes Bay using a 'NO WRONG DOOR' approach.
The referral process will not change, a person may self-refer, or any other person may refer on behalf of that person if they have the person's consent.
It is important to know that you can continue to send referrals to NASC Hawkes Bay (NASC.HB@hbdhb.govt.nz) and they will redirect your referral to the National Referrals centre, however the NRC is encouraging referrals to be made electronically via www.nrcentre.co.nzor email referrals can be made to referrals@nrcentre.co.nz
For those referrals that do not meet the eligibility criteria for Ministry of Health funded disability support services, you will be referred to a local organisation who will work with you to help you and your family connect with the support you need based on the same 'NO WRONG DOOR' approach.
MSD has been engaging with the Disability sector over the last month or so on how best to use the $5m Care in the Community Disability Welfare Fund (the Fund) announced by the Hon Carmel Sepuloni on 5 May 2022.
This fund is to address the welfare needs of the disabled community and the wellbeing of disabled people and whānau impacted by COVID-19.
The Fund is administered by the Ministry of Social Development (MSD), and in line with the ‘nothing about us without us’ approach, MSD has engaged closely with the disability sector and other government agencies to determine the best options for the use of this funding.
Four key needs were identified through this engagement and meeting one or more of these needs is the basic criteria for the distribution of funds from either of the two new funds developed. These needs are:
Connecting to social networks, communities, and identities
Support for the whānau of a disabled person
Precautions to prevent exposure to COVID-19, and
Connecting with support networks.
The $5m fund has been split between two new funds:
The Direct Support fund - enables disabled people and whānau to access support from providers who have received the funding.
The Provider-led Innovation fund (community support) - enables providers to deliver a project, product, or initiative to a target community.
