A Message From Dr. Young

Dear Valued Patients, Friends and Colleagues,

We hope that you are having a wonderful summer, enjoying the blue skies and shimmering waters! As we head towards Fall, many of us are planning rejuvenation treatments to lift away sun damage.

With many patients inquiring about non- or minimally invasive procedures designed to reverse the signs of aging, we are in a new era of ‘prejuvenation’. Dr Ken Arndt in 2013 coined the term prejuvenation—“to prevent the loss of youth”. While rejuvenation focuses on recovery, prejuvenation focuses on delaying aging symptoms for as long as possible. Prejuvenation treatments are designed to increase collagen production, support elastin function, and preserve facial volume. Prejuvenation is the act of initiating a series of thoughtfully planned steps that will prevent or impede the changes associated with the appearance of “aged” skin (i.e., lines, wrinkles, brown spots, and thin, dry, crepey skin).

These steps include a comprehensive approach including an anti-aging skin care regimen (consisting of antioxidants, sunscreen, and retinoids), laser resurfacing procedures to reduce effects of sun damage and stimulate new collagen production, neuromodulators for prevention and softening of wrinkles, soft tissue fillers consisting of hyaluronic acid for artful replacement of volume loss and non-surgical lifting of saggy skin, and skin tightening procedures including radiofrequency microneedling which induce preventative tightening of collagen in the skin, for years to come.

Prejuvenation will not only maintain the health and aesthetics of youth, but also enhance it in the most natural way possible. If preventative skin care is started early in life, skin changes associated with aging, which are mostly preventable, will simply not evolve. The concept of preventative skin care may in fact be necessary as photodamage begins earlier than we think, in childhood.

No Clear Risks For Venous Thromboembolism (Blood Clots) Associated With Patients Treated With JAK Inhibitor Therapy For Atopic Dermatitis (Eczema), Systematic Review And Meta-Analysis Find

HCPlive (8/24, Smith) reported, “No clear risks for venous thromboembolism (VTE), a type of blood clot, were associated with patients being treated with Janus kinase (JAK) inhibitor therapy for atopic dermatitis,” researchers concluded in a 17-study systematic review and meta-analysis encompassing 466,933 patients. The findings were published online in JAMA Dermatology.
 
The new oral Janus kinase (JAK) inhibitors (Upadacitinib and Abrocitinib) are very effective in treating moderate to severe atopic dermatitis in the appropriate patients. Although the FDA labeling includes a risk for venous thromboembolism (VTE), blood clots are uncommon in eczema patients treated with this class of medication. – Dr Young
 

Dupilumab Appears Effective In Treating Pediatric And Adolescent Atopic Dermatitis, Research Suggests

Dermatology Advisor (8/24, Goldberg) reports, “Dupilumab was efficacious and well tolerated regardless of sex, age, or race/ethnicity in treatment of adolescent and pediatric atopic dermatitis,” investigators concluded in the findings of an 89-patient study published online ahead of print in The Journal of Allergy and Clinical Immunology: In Practice.

With The FDA approval of Dupixent (dupilumab) to include children as young as 6 months with moderate-to-severe atopic dermatitis (eczema), we are finally able to treat children with moderate to severe eczema, recalcitrant to topical therapy, safely and effectively. – Dr Young
 

Patients With Rosacea Consider Persistent Facial Redness As Most Bothersome Symptom Of Their Condition, Survey Finds

Healio (8/25, Forand) said that most patients with rosacea “report persistent facial redness as being the most irritating and bothersome symptom of their condition, according to a survey conducted by the National Rosacea Society” to which 986 patients responded. The findings from the survey were posted to the society’s website.
 
Besides optimizing medical treatment for rosacea and avoiding trigger factors, I find the use of vascular lasers including the pulsed dye laser and KTP laser (which target and treat excessive facial veins selectively) to be effective and safe in reducing persistent facial redness in patients with rosacea. – Dr Young

Dermatologists Increasingly Prescribing Low-Dose Minoxidil To Address Hair Loss

The New York Times (8/18, Kolata) reported “a growing group of dermatologists is offering low-dose minoxidil pills” to treat hair loss, but “the treatment remains relatively unknown to most patients and many [physicians].” The treatment “has not been approved by the Food and Drug Administration (FDA) for this purpose and so is prescribed off-label.”
 
Minoxidil’s “use as a hair-growth treatment was discovered by accident decades ago,” as higher doses of minoxidil “were being used to treat high blood pressure, but patients often noticed that the pills prompted hair growth all over their bodies.” A topical preparation was first marketed in 1986. For several decades, minoxidil has been used as 2% and 5% topical solutions and, later, 5% foam for the treatment of a variety of hair loss conditions in both men and women, although it was FDA approved only for androgenetic alopecia. Minoxidil causes a shortening of the telogen (resting) phase and lengthening of the anagen (growth) phase with a progressive growth in hair follicle diameter and length.
 
Although topical minoxidil is an effective treatment option for hair loss, many patients find it difficult to use because of the necessity to apply the medication twice a day, undesirable hair texture, and scalp irritation. There is a temporary shedding period that occurs after the initiation of topical minoxidil application. If unaware of this transient adverse effect, patients may discontinue use prematurely.
 
Low-dose oral minoxidil has promising results as an effective and safe treatment option for a variety of hair loss conditions. Androgenetic alopecia was the most studied condition in both men and women. In a multicenter study of 1404 patients to address the safety of low-dose oral minoxidil for hair loss, it was found to have a good safety profile. Only 1.7% of patients required discontinuation of the drug due to adverse effects. By far, the most common adverse effect was hypertrichosis (facial and body hair growth), which was reported as mild and easily manageable. Although infrequent, patients should be aware of potential adverse systemic effects of low-dose oral minoxidil including lightheadedness, palpitations, headaches, and lower leg swelling. In the appropriate patient population, oral minoxidil is an effective treatment option for hair loss in healthy young patients who are having difficulty tolerating the topical formulation.
