Free K-12 COVID-19 Summer School and Camp Testing

• Tests are unproctored and should be administered at home. 
• Each test kit includes two tests.  
• Tests will provide results within 15 minutes.
• These tests can be used by children as young as 2 years old. 
• To order these tests, use fill out the Rapid Testing Supply Order Form for Eligible Organizations on Google Forms.
• The kits will be available throughout the summer and can be ordered and distributed by schools or districts. 

• A trained staff member will administer the tests on-site at participating schools.
• Test results must be reported using the reporting application SimpleReport. Find SimpleReport enrollment Instructions under the header “How to Report COVID-19,” Option 2 on the CDHE COVID-19 Reporting Requirements webpage.
• Schools interested in receiving these tests must fill out the Opt-In Form for Free COVID Tests for Summer 2022 on Google Forms. 
• Enrollment for this testing option will end on Friday, July 8, at 5 p.m.
