As a new student,
you are required to complete the
MSU COVID-19 vaccine verification form

to report your vaccination status to the university
BEFORE you can register for Fall semester courses. 

To complete the form, you either need to: 
be fully vaccinated and boosted* against COVID-19
with a World Health Organization approved vaccine, OR
apply for a medical exemption/deferral,
before you can register for your Fall semester courses.


*A booster is a dose of vaccine given as a refresher to the original vaccine
usually a few months after the original vaccine. You can read more
about boosters on the CDC COVID-19 Vaccine Boosters Site

 

You must complete the vaccine verification form online
before June 6, 2022. You DO NOT need to be vaccinated to complete the vaccination form. 


 
COMPLETE THE VACCINE VERIFICATION FORM
If you are
FULLY VACCINATED AND BOOSTED
against COVID-19, follow these steps:
 

1. Enter your initial vaccination information 

  • Enter your information using MSU's vaccine verification form. Ensure you are using a supported browser (Chrome, Firefox or Edge)
  • Provide the dates of vaccination and the manufacturer of the vaccine (Pfizer, Moderna, etc.)
  • Click “Submit” to complete the requirement.  
If you are 
NOT FULLY VACCINATED AND BOOSTED
against COVID-19, or are 
VACCINATED BUT NOT WITH A WORLD
HEALTH ORGANIZATION APPROVED VACCINE,

follow these steps:
 

1. Create and save a Word doc or PDF that includes the following information: 

  • Document name: Medical Deferment Statement
  • First name
  • Last name
  • MSU NETID (part of your MSU email before @msu.edu)
  • Your anticipated date of arrival in the US
  • A statement that you will update your vaccination status if it changes prior to your arrival in the US and that you get the required vaccinations upon arrival in the US 
2. Login to thevaccine verification form. Ensure you are using a supported browser (Chrome, Firefox or Edge)

3. When you see the question Have you been vaccinated for COVID-19 with an FDA-authorized or WHO-approved vaccine? Select “No, I have not”

4. On the next screen select "Request Exemption" 

5. Select "Medical Exemption" 

6. Upload your medical deferment statement where it asks for supporting documentation

7. Submit the request 

You will receive notification of approval in three business days.

Anticipated date of arrival in the US: I plan to arrive on August 22, 2022
I am requesting a medical deferment and will get vaccinated when I arrive in the US. 
IMPORTANT:

You DO NOT need a statement from a medical provider/doctor to request a deferment as an international student. 

You DO NOT need to select any of the options for a medical exemption. 


You will have a CDC (COVID) Service indicator "hold" on your student account that will prevent you from enrolling in classes. This service indicator will be removed after you complete the vaccine verification form
