UPDATE: VTAT has selected a task force and are in the progress of creating a report!

The task force will be responsible for creating a report to present to the Texas Board of Veterinary Medical Examiners including suggestions for opening the practice act for changes in delineated tasks for LVT’s and proper use of nomenclature. 
WE WANT TO HEAR FROM OUR MEMBERS!!!

With SWVS's annual conference quickly approaching this is your chance to have your voice heard! We will be creating a report to present at the Paraprofessional committee meeting about changes that support staff of Texas want to see. Help be the change and get involved!

Case Study Example
On presentation: Intact 1 yr 4 month old female bull dog showing signs of labor (mucoid and red-tinged vaginal discharge) but no active pushing on December 14, 2016 at ~ 1:00 A.M.
History: Patient had been inseminated in Austin three days in a row (no specific date given) with another vet for her first pregnancy. Due to the fact that they were uncertain if she was ovulating it was unclear how far along she was. On November 30, 2016 owners brought patient in for pregnancy confirmation and cesarean consult.                      
              Plan:
  • Perform a PE to confirm labor is occurring
  • Place catheter to run fluids
  • Ultrasound to confirm puppies heartbeats
  • Prep for cesarean section
  • Perform c-section
  • Stimulate and clear puppies
  • Recover mother and reunite with pups once she is recovered
  • Monitor them the rest of the day and if doing well , release to owner at 3:00 P.M.
              Physical Examination:
  • Patient is BAR
  • Vulva is swollen and has mucoid discharge; no fetuses in the birth canal
  • Catheter is placed after multiple attempts
  • Ultrasounds confirm active puppies
Assessment/ Diagnosis:
  • Patient is in early labor
  • Planned C-section is discussed with owner 
    • Induce with Propofol
    • Maintain with Isoflurane
    • Peri-operative drugs/antibiotics-Buprenorphine
    • Administer fluids
    • Record TPR every five minutes
Postop:
  • Cesarean section-a ventral incision was made using a #10 blade; after the uterine horns were exteriorized one at a time, a longitudinal incision was made in the distal uterine body; one at a time the fetuses were milked towards the incision and removed and handed to assistant for membrane removal after the umbilical cords were clamped with hemostats; the incision in the uterus was closed with a 2-0 PDS with a simple continuous pattern; the abdomen was closed in two layers: the body wall closed with an 0 PDS with a simple continuous pattern and the subcutaneous tissue and skin was closed with 2-0 Biosyn with a subcuticular pattern and surgical glue.
  • There were a total of four fetuses- three were successfully revived and one was nonviable/poorly developed
Treatment:
  • Monitor mother and pups now that they are united
  • Dispense:
    • Rovera (Carprofen) 75 mg Tab                  (10 Tabs)
      • Give 1 tablet PO every 12 hours for 5 days with food and discontinue if any vomiting or diarrhea
    • Cefpodoxime (Simplicef) 200 mg              (10 Tabs)
      • Give 1 tabler PO every 24 hours for 10 days
    • Buprenorphine Inj                                       (3 mls)
      • Give 1 mL IV q8h
Note:
               This was a very exciting procedure for me to be a part of since it was my c-section that was able to watch and assist with. It was nice to see how everyone worked together to care for the mother and the puppies. It was also eye opening to see how real practice has similarities and differences from what we learned in class. I was able to restrain the patient and be one of the circulating nurses in surgery including running for supplies and monitoring intraoperatively. I was also the one that got to continue to monitor the mom and pups the rest of my shift. Two nights later one of the puppies was brought back as emergency for not breathing. By the time they arrived the puppy was already deceased. There were no signs of trauma or congenital issues. It is believed that maybe the mother had laid on the puppy, but the owner did not witness anything to confirm.
