Dear Kindergarten Parents,
As we prepare for the start of the 2022/23 school year, we want to make sure we have all of the necessary health forms completed for your child.
Report of Health Examination for School Entry
All incoming Kindergarten students are required to submit a "Report of Health Examination for School Entry". If your child has recently had a physical examination, your doctor's office may fill out this form without seeing your child again. The form can be found HERE. The form needs to be returned to me no later than August 1, 2022. Your child must have this form completed prior to the first day of school.
Medication Administered at School
If your child needs any medication administered at school (for example, an inhaler, an EpiPen, etc.), THIS FORM must be completed by your doctor. This completed form needs to accompany the medication in its original packaging. The form and medication should be brought with you to Registration Day (Monday, August 15).
