Community Connector Service
Supporting the Health and Disability Community Impacted by COVID-19
The Needs Assessment and Service Coordination Service is for people with a physical, intellectual and or sensory impairment, or a disability that is likely to last longer than 6 months and reduces their ability to function independently.
As part of the Ministry of Health's Disability Support Service initiative to support NASC, a National Referrals Centre is being piloted and is now working alongside NASC Hawkes Bay using a 'NO WRONG DOOR' approach.
The referral process will not change, a person may self-refer, or any other person may refer on behalf of that person if they have the person's consent.
It is important to know that you can continue to send referrals to NASC Hawkes Bay (NASC.HB@hbdhb.govt.nz) and they will redirect your referral to the National Referrals centre, however the NRC is encouraging referrals to be made electronically via www.nrcentre.co.nzor email referrals can be made to referrals@nrcentre.co.nz
For those referrals that do not meet the eligibility criteria for Ministry of Health funded disability support services, you will be referred to a local organisation who will work with you to help you and your family connect with the support you need based on the same 'NO WRONG DOOR' approach.
On 1 July 2022 Te Whatu Ora – Health New Zealand and Te Aka Whai Ora – Māori Health Authority became Aotearoa’s new national health authorities. We don't yet know what impact this will have on us or on the wider diabetes community. However, as diabetes is a major health priority, we hope the challenges will be addressed in the new Health Plan.
We're excited to share news of the mahi being done in Waikato; really positive change is happening in the diabetes community there as a result of our and others' work. Plus some food tips - reading food labels, accessing our native rongoā, and of course a delicious recipe.
Capture the Moment – our new photography competition for New Zealanders who have an intellectual disability or an intellectual disability and autism.
The theme of the inaugural competition is "in your community" and we'd love to see examples of what community means to you wherever you live in Aotearoa New Zealand.
This means it could be a photo of taken at the park, beach or even at home.
Entries are open now and close at 5pm on Sunday 31 July 2022.
The Playful Place is introducing a a second Food Explorer Group on Saturdays 12.30pm-1.15pm. This group will suit kids around 9 years old who are sensitive, introverted and may need an extra bit of support to feel safe in groups.
Nationwide Health and Disability Advocacy Services
The Nationwide Health and Disability Advocacy Service offers free, independent, and confidential advice and support to help you resolve issues with health and disability services.
Our advocates are here to let you know about your rights, answer your questions and talk through your options for making a complaint. We will support you through the whole process.
For the period 01 July 2020 - 30 June 2021, the service closed 2675 complaints – 81% of those were closed within 3 months. 82% of all closed complaints were resolved or withdrawn by the consumer / complainant. During that same period, we facilitated over 1,200 education sessions and nearly 3,800 network meetings and visits.
We are pleased to be able to send out this information and to keep the community informed about what’s going on. Please continue to send us your flyers, and any information you have about cool stuff happening here. Don't forget to add your contact details for readers who want further information about your event.
We also appreciate any feedback you have about this newsletter. Tell us what we got right, got wrong or just let us know what you want to see.
