At HC365 we hope that you are enjoying the good weather, and the activities that come with it. As you might be aware, our After Hour On-Call System has changed. To provide better care through our Call Centre, we have enhanced our nursing team with these additional HealthCare365 Nurses.

What is the After Hours On-Call service?
- This is a Telephone Nursing service after regular office hours.
- Hours of operation are from 5pm - 8am and 24 hours/day on weekends and holidays.
- THIS IS NOT an EMERGENCY Medical service. In an emergency call 911.

What do Virtual HC365 Nurses do?
When you call our main number (416) 642-9160 after hours, you will be transferred to a Virtual HealthCare365 nurse, or you will be called back within 2 hours.
The nurse will assess your concern using an established protocol to guide data collection, triage (determination of urgency), provide direction where and when to seek in person care, and give advice on self-care measures.
The Virtual Nurses are supported by your On-Call Medical Team - Tanya Radforth NP and Dr. Vera Kohut, who can be accessed by the nurses as required for additional direction, advice, or urgent prescription renewals.
There will be no direct transfer of calls to the On Call Medical Team. Please DO NOT SEND EMAILS after hours about medical concerns.
