A query can be a powerful communication tool used to clarify documentation in a health record and achieve accurate code assignment. Querying the physician has become a common communication and educational tool for coding departments. An effective query process aids the hospital’s compliance with coding rules and serves as an educational tool for providers, and coding professionals, on the physician side. This session will include common missed physician query opportunities and key indicators to look for in the chart and also when to query and when not to query for infectious (sepsis), cardiology, respiratory, renal, and neurology complications.
