Dear Y Employee,

As a Y employee working in a State program (A+, Uplinks, 21st Century, Kalihi Y Counseling) who is still in the process of being fully vaccinated OR who has chosen not to vaccinate, you are required to upload weekly COVID-19 test results no later than 2 pm every Wednesday.

Currently, the State is not allowing employees in State programs to use COVID 19 at-Home test kits, so you will need to secure, on your own, COVID-19 tests that are currently offered at sites across the island. Please plan ahead as most of these sites require advance registration.

You will need to take a photo of your weekly test results and upload by 2 pm every Wednesday.

Please remember, If you do not take and load your weekly test results, per our Y and State mandate requirements, you will not be able to report to, or enter your work site. As a result, you will be placed on unpaid leave until you submit a valid test result taken within 7 days of your return to work. Please be sure to take and upload your test results each week before the deadline.
