The Department of Health (DOH) just announced that they will be mailing a set of 4 rapid antigen COVID-19 home test kits for free to anyone who lives in the zip codes listed below. Each kit contains 2 tests, so this would be enough for 8 weeks of weekly testing.

However, the DOH is limiting the set of 4 test kits to the first 125,000 who order online. Please order yours as soon as possible if you live within one of the zip codes listed below.

The site is experiencing heavy online traffic, keep trying and you may need to be patient as the system cycles through all of the people trying to place their orders.

The test kits are free. You can use the results from your test kits to meet our Y's weekly testing requirements if you have chosen to test or are still in the process of becoming fully vaccinated. Please scroll to the bottom of the page for information about how your data will be kept private by the DOH.

  3. Click on the red box link that reads: "Send me my tests."
  4. Fill out the order form.
  5. The website is currently experiencing high volume, please keep your screen open until your order form goes through. Once it goes through, you will receive a confirmation and also an invite to order an extra set of 4 test kits for a friend or neighbor.
  6. Pro Tip: When submitting your address, you may receive a message that your address is not valid, keep trying and perhaps spelling out the directional parts of your address: e.g. South versus S. As mentioned, the system is experiencing delays due to heavy traffic.

While we do plan to provide those Y employees with home test kits who are choosing to test weekly or are in the process of being vaccinated, we are requesting that you apply to receive the free test kits from the DOH if you live in one of the eligible zip codes listed above.

This will help greatly in managing our inventory of test kits during this time when currently, there is a nationwide shortage of home test kits available to businesses and consumers.

We will be following up with you later this week by email with additional details on how to use home test kits for your weekly testing requirements.

How the Department of Health Keeping Your Data Private [This is information provided by the DOH]
Your privacy is a priority for the DOH's Say Yes! COVID Test program.

For online test kit orders, the DOH just needs your name and address to be able to mail your test kit to you. The DOH will not share this information with anyone outside of this program. Providing your phone number is optional if you would like to receive text messages about test kit delivery and reminders about testing.

Any data that is shared by the DOH outside of the program will be anonymous and not tied to your name, address, phone number, or any information that identifies you individually.

Do I have to report my test results?
No. But you are invited to share your test results with the public health department through their Digital Assistant test reporting tool. There are many benefits to you and to your community when you confidentially report your results to public health. There is a link on the DOH Say Yes! COVID Test program website to opt into the Digital Assistant test reporting tool.

Who has access to my test results?
If you opt into sharing your test results via the DOH Digital Assistant test reporting tool, data collected through the Say Yes! COVID Test Digital Assistant tool will be de-identified and may be shared with researchers and support teams working on the Say Yes! COVID Test initiative. Any data that is shared outside the program will be anonymous and not tied to your name, address, phone number, or any information that identifies you individually.

Optional Health Department Reporting
If you choose to participate in the optional public health reporting, you will be asked to share additional information that identifies you. This information, along with the results of your test, will be shared with the health department with your explicit permission.

What data will be collected if I use the DOH Say Yes! COVID Test Digital Assistant tool?

The DOH Say Yes! COVID Test Digital Assistant tool captures general data about the frequency with which the Digital Assistant tool is used, any results recorded, and any images of test results you take using your smartphone camera. If you use the tool on behalf of yourself and a family member, you can choose a nickname to maintain a diary of your results individually for your convenience. The tool does not use any identifying information about you outside the program. Any data that is shared outside the program will be anonymous and not tied to your name, address, phone number, or any information that identifies you individually.
