Digital COVID-19 Vaccine Record

This portal allows you to enter a few details and get a link to a QR code and digital copy of your COVID-19 vaccination record. If you want to share your proof of vaccination, you can use either the electronic version you’ll get from the portal or the card you were given at the time of vaccination.

If you are a parent or guardian and have multiple vaccine records associated with a single cell phone number or email address, enter each digital vaccine record request separately.

The portal provides only a digital copy of your vaccine record. If you received your vaccinations from a federal agency (e.g., Department of Defense, Indian Health Services, or Veterans Affairs), you need to reach out to those agencies for assistance with your vaccination record.

Get your digital vaccine record here.

Mobile Vaccine Request Form

Offered by the County of Marin and Marin Health and Human Services.

Are you still unvaccinated? Is it hard for you to get to a vaccine site?
You can request a mobile visit here.

Fear, Hesitancy, Lack of Access – Overcoming Barriers to Vaccines in the African American Community

Presented by The California Department of Aging, California Black Media, & Ethnic Media Services

WHEN
Tuesday, September 21, 11:00 am – 12:00 pm PT | 2:00 pm - 3:00 pm ET

WHERE
Register in advance for this meeting: Click here After registering, you will receive a confirmation email containing your individual link and information about joining the meeting.

WHY
Hesitancy about the health care system, not hesitancy about vaccines, is part of why vaccine rates remain low among African Americans, as are fears about the vaccines and misinformation about the COVID pandemic itself. Speakers will discuss approaches that are slowly increasing Black vaccination rates in California, especially among young adults, and some will share the reasons why they delayed getting vaccinated until very recently.

WHO
  • Dr. Oliver T. Brooks, Chief Medical Officer, Watts Healthcare Corp., Los Angeles
  • Alva Brannon, recent vaccine recipient
  • Rev. Shepard, Pastor, St. Paul AME Church, San Bernardino
  • Christopher Hargrove-Thompson, student/rapper, recent vaccine recipient
  • Cheryl Brown, former California State Legislator and co-founder, Black Voice News
  • Sabrina Saunders, One Accord Project, African American Clergy & Churches (Invited)
