Care Navigation
(Effective Patient Engagement)  

 

Tuesday, 19th October 2021 (12:00 - 16:30)

Wednesday, 3rd November 2021 (12:00 - 16:30)

Only available to General Practice staff in Suffolk & NE Essex

The key objective of engaging the patients and signposting is to help patients access the right care at the earliest opportunity. It is a new system of ‘triage‘, carried out at the first point of contact with the GP Practice by non-clinical staff.

Learning Outcomes

  • Skills and confidence to deliver signposting
  • Understanding what signposting is for
  • Benefits to patients
  • Benefits to the practice
  • Using protocols
  • How to make referrals to allied healthcare professionals and other internal clinical team members such as Physician associates, Physiotherapists and Paramedics
  • Understand the new roles under the ARRs scheme and how to engage them
  • Receptionist role in engaging the patient
  • Communication with the patient
  • Patient safety
  • effective messaging for patients
  • coordinating Websites/ Facebook/ Telephone messaging
  • New consultation types (Telephone triage , Online consultation, Group consultation)

Joining instructions will be emailed directly to delegates by PMA along with slides and handouts and any pre-session work  to allow delegates to print these for reference or the exercises that will be an integral component of the web workshop format.

Due to popular demand, and very positive feedback, The Suffolk & NE Essex Training Hub has commissioned additional fully-funded sessions this Autumn.  See below for details on how to register.



“One in six school-aged children has a mental health problem. This is an alarming rise from one in ten in 2004 and one in nine in 2017.” (NHS Digital, 2020)  As numbers grow exponentially, GP surgeries are seeing more and more self-injury in teenagers and younger children.

 

  • 1.5 hours online training course, including 1/2 hr Q&A session.
  • 13:00 - 14:30 via Microsoft Teams.  
  • Details of how to join event will be sent to attendees 2 days prior to event.

Physician Associate Apprenticeship – Employer Engagement Webinar – 28th October 2021, 12pm – 2pm


Health Education England are supporting a collaborative, national implementation of the Physician Associate Apprenticeship to ensure apprenticeship provision meets the needs of all employers and potential apprentices.  This webinar is suitable for employers who are considering implementing the Physician Associate Apprenticeship within their organisation and would like information around practical steps and actions needed to ensure effective implementation.

Developing strategies to make effective use of the Personalised Care ‘additional’ roles in Primary Care

What is the support offer?  
Time with experts in social prescribing to help you develop clear strategies to make the most of these roles
.
This support offer is currently only available until March 2022 so early contact is advised.  The experts will work with you on “what matters to you” , flexing their support to meet your needs – this might be a single meeting to answer some key questions or test some ideas, to 2 - 3 meetings or a workshop involving wider members of the PCN team to help raise awareness of the three personalised care roles and how they fit as part of a multi-disciplinary team approach to care. We will shape the offer with you to meet your needs

How are we making the new Primary Care landscape work for us?
How can Primary Care operate effectively in the wider system?

Leadership and Lifelong Learning (LLL) in the East of England Region has been running Leadership Collaboratives for Primary Care staff working in individual practices and at Primary Care Network level since 2018. Six cohorts have had leadership development support so far, latterly online due to Covid-19.

At both upcoming events, the aim will be to provide a safe space for people to come together and have conversations about key issues for them, alongside hearing about current and new support available. At the same time, the region can demonstrate the breadth of support on offer.
