HealthCare Safety Investigation Branch (HSIB) Latest report on Intrapartum stillbirth.
HSIB have published a report this week - Intrapartum stillbirth: learning from maternity safety investigations that occurred during the COVID-19 pandemic 1 April to 30 June 2020
In 2019, 24 stillbirth referrals were made to HSIB that met maternity investigation programme criteria between 1 April and the end of June. During this same period in 2020 there were 46 stillbirth referrals.
This latest HSIB report:
- Describes the context within which the intrapartum stillbirths took place
- Seeks to understand why we have seen this increase
- Identifies learning to improve patient safety across the healthcare system
- Shares that learning with NHS trusts across the country.
HSIB have made eight safety recommendations, five to NHS England and NHS Improvement, one to RCOG, one to NHSX, and one to DHSC.
A response to the safety recommendations is anticipated within 90 days of publication of the investigation report.
New guide published to support mental health in maternity and neonatal settings
NHS England and NHS Improvement have published a new guide this week for healthcare professionals and commissioners to support mental health for new, expectant or bereaved mothers.
It describes the underpinning principles of good practice for supporting good mental health and psychological wellbeing in maternity and neonatal settings. It also considers the role of specialist maternal mental health services (MMHS) in supporting this function.

Baby Buddy 2.0 beta - new version of the NHS-approved Baby Buddy app - available to be used and tested by the public.
A new beta version of Baby Buddy is available and a request has been made for user feedback to inform improvements and optimisation ahead of a public launch.
The beta Baby Buddy app can be downloaded here, and you can share can your feedback here.
The new version of the app includes:
- Personalised daily information for mothers, fathers and other non-birthing parents to the 1st birthday
- Choice of Place of Birth feature with Birth Choice UK data from every hospital across the UK
- Digital Personal Care & Support Plans

Baby Lifeline’s National Maternity Safety Conference 2021
The second annual National Maternity Safety Conference was hosted by Baby Lifeline on the 9th September.
The theme for the in person conference at the Hilton Birmingham Metropole was Getting Safer Faster.
The conference had a wide range of speakers including clinicians, parents, researchers, experts, charities and politicians working across the UK to improve maternity and patient safety.

Webinar: Maternity Incentive Scheme – Yr 4 (CNST) – safety action 2: submitting maternity MSDS data.

A webinar to give guidance and support for the successful completion for safety action 2 is planned for an audience including digital midwives, MSDS data submitters, Regional Chief Midwives and HOMs and DOMs
The webinar will give an introduction to safety action 2 and outline key available national resources such as:
- Scorecard
- Data Quality Submission Tool
- Technical Guidance Videos
- Drop In sessions
- On-line resources
Presenters from NHSE&I, NHSD will be available to answer any queries or questions that you may have.

Maternal Mental Health Services – implementation event

The national perinatal mental health team will be hosting an event for those involved in the design and delivery of MMHS, in Early Implementer sites, Fast Follower sites, and beyond! Hear the latest programme updates and learn from what sites have been doing. We will also discuss next steps and how to prepare for transformation funding shifting to CCG baseline funding from 2022/23.
