It’s hard to believe that it has been more than a year since Young Dermatology opened. This has been an eventful year full of blessings. We have learned that sometimes in life, it isn’t only what you can do at a given moment in time that defines or drives you, but rather, it is what you can’t do that makes you realize you can be limitless in your endeavors.

We are so grateful for the love and support of all our patients, colleagues and friends during these most challenging times. Thank you for believing in us.

Heliocare® Advanced contains Nicotinamide and Polypodium leucotomos, oral antioxidants that enhance photoprotection

Non-melanoma skin cancer (most commonly, basal cell carcinoma and squamous cell carcinoma) is the most common form of cancer in the United States. Behavioral changes, such as the use of hats, protective clothing, and sunscreens are effective and well-known means for preventing skin cancer. When used in conjunction with topical antioxidants and sunscreens, oral antioxidants further potentiate sun protection.

The prevention of common skin cancers and precancers is possible by taking oral vitamin B3 supplement (also called Nicotinamide or Niacinamide). Nicotinamide reduced the rate of new squamous-cell and basal-cell skin cancers by 23% compared with placebo after 1 year among patients at high risk for skin cancer. Nicotinamide also reduced the risk for developing actinic keratosis, a common precancer of the skin (Chen, AC et al. A Phase 3 Randomized Trial of Nicotinamide for Skin Cancer Chemoprevention. N Engl J Med 2015; 373:1618-1626).

Polypodium leucotomos is a South American species of fern in the family Polypodiaceae. Clinical research has shown that it has antioxidant and photoprotective properties and taken orally provides protection against the harmful effects of ultraviolet (UV) radiation from the sun by enhancing the activity of our own antioxidant systems responsible for blocking the formation of reactive oxygen species (Edlich RF et al. Photoprotection by sunscreens with topical antioxidants and systemic antioxidants to reduce sun exposure. Journal of Long-Term effects of Medical Implants 2004;14(4):317-340; Middelkamp-Hup MA et al. Oral Polydium leucotomos extract decreases ultraviolet-induced damage of human skin. J AM Acad Dermatol 2004;51:910-8).

Some Lip Balms Can Make Dry Lips Worse
According to USA Today (12/8, Moniuszko), “not all lip balms are made the same, and some can even make a dry lip situation worse, experts say.” For example, while the products “can temporarily soothe irritated lips, they often have chemicals that may cause further irritation...(including) flavors, fragrance and preservatives that can dry and irritate” the lips. One “rule of thumb from the American Academy of Dermatology (AAD) is, ‘if your lips burn, sting or tingle after using a lip product, stop using that product.’” People should also resist the temptation to lick their lips, because “‘as saliva evaporates, your lips become drier,’ according to the AAD.”

I would recommend patch testing to evaluate for potential allergic contact dermatitis to one of the active or inactive ingredients in your lip balm if you have persistent irritation with chronic application. – Dr Young

Combination Of 1927 Nm Fiber Laser Therapy, Tretinoin 0.05% Lotion May Result In Improvements In Patients With Facial Hyperpigmentation And Photodamage, Researchers Say Healio (12/1, Volansky) reports, “The combination of 1927 nm fiber laser therapy and tretinoin 0.05% lotion showed significant improvements in patients with facial hyperpigmentation and photodamage,” investigators found in a “single-center, prospective, double-blind, evaluator-blind, non-randomized, vehicle-controlled study” in which “a cohort of 41 women underwent one session of 1927 nm thulium fiber laser treatment.” The study also found that “laser therapy with tretinoin failed to show improvement over laser therapy without tretinoin.” The findings were published online in the Journal of Drugs in Dermatology. I have found 1927nm non ablative fractionated laser to be effective in the treatment of photodamage, freckles and actinic keratoses (precancerous lesions) with lower risk of post-inflammatory hyperpigmentation. – Dr Young

Vitamin D, omega-3 supplements reduced risk for incident autoimmune disease
The use of vitamin D3 or omega-3 fatty acid supplements over a 5-year period may reduce the incidence of autoimmune disease in older adults by 25% to 30%, according to data presented at American College of Rheumatology (ACR) Convergence 2021.

“In past ecologic observations, inflammatory bowel disease, multiple sclerosis and type 2 diabetes have been shown to be more prevalent at northern latitudes, where circulating vitamin D levels are lower,” Karen Costenbader, MD, MPH, director of the Lupus Program at Brigham and Women's Hospital, told attendees at the virtual meeting. “Both high plasma 25-OH vitamin D and high residential UV exposure were associated with a decreased risk for rheumatoid arthritis among women in the Nurses’ Health Study in our past work.”

I recommend taking vitamin D 2500 units a day in high risk individuals, especially in the winter months in Seattle. – Dr Young

Antioxidants in foods that combat photoaging:
Lycopene - tomatoes, tomato paste
Polyphenols – green tea, pomegranate, berries, flaxseeds, cocoa powder, herbs and spices, nuts, vegetables, olives
Flavanols – cocoa, onions, kale, grapes, red wine, tea, peaches, berries, tomatoes, lettuce
Ellagic acid – raspberries
Genistein – soy
Curcumin – turmeric
Quercetin – onions, kale, cherry tomatoes, broccoli, blueberries, apples
Resveratrol – grapes
 
Foods rich in vitamin A that may decrease squamous cell carcinoma risk:
Carotenoids – yams, kale, spinach, watermelon, cantaloupe, bell peppers, tomatoes, carrots
Retinol – salmon, beef liver, dairy products such as milk, butter, cheddar cheese, eggs, fish, cod liver oil, shrimp
 
Oral supplement that enhances DNA repair (decreases non melanoma skin cancer risk by 23%, actinic keratoses/precancerous lesions risk by 11%):
Nicotinamide 500mg twice daily (Avoid in patients who are pregnant, have low platelets, kidney disease, patients on carbamazepine, niacin and statins.)
