Dear PTA Leaders,

The Workers’ Compensation Annual Payroll Report for the policy year ending January 4, 2022, is now available to complete! This form and any related surcharge that might be due must be submitted to Association Insurance Management (AIM), California State PTA’s Insurance Broker, no later than January 31, 2022.

The Workers’ Compensation Annual Payroll Report is a form that every PTA in the state is required to submit directly to AIM for insurance purposes. It is used to indicate if your PTA paid any individual (including employees and independent contractors) for services, and the amounts you paid, if any. The form must be submitted each year, even if your PTA did not pay anyone.

The form is available in English and Spanish and can be completed and submitted online. Alternatively, a paper version can be printed and mailed. If you print and mail the form, please be sure to mail it to AIM, NOT your council or district. To access the form, simply click on the method you would like to use to complete and submit it.
Please note: This process only applies to the Workers' Compensation report and any related surcharge that might be due. Every unit, council or district should still pay their regular insurance premium through channels. Check with your council or district for local due dates.

What is new this year?
Companies no longer need to be reported as payees in your Workers' Compensation Annual Report. Only individuals paid directly by the PTA for services need to be reported. This includes employees, individuals you issue 1099s to, and ANY other individual that was paid for services directly by the PTA.

Where can I find instructions on completing and submitting the form?
To access one of resources listed below, simply click on the name of the resource. You may want to review these instructions prior to completing the form. You can find these resources and others under the Workers’ Comp Annual Payroll Report Process heading with the Insurance Coverage and Service section of the California State PTA’s website.

Who should I contact for questions?
If you have any questions about this process, the form, or Workers’ Compensation coverage in general, please contact AIM for assistance by emailing capta@aim-companies.com or by calling (800) 876-4044 or (214) 360-0801. AIM has Spanish-speaking representatives, and is available 24/7.
